Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge. Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes.
Responsibilities:
1. Evaluates patient needs, provides case management.
2. In collaboration with the M.D. /CNS, provides direct patient care to patients, including, but not limited to, medication management follow-up and decision-making about follow-up care, crisis stabilization, medication changes, and wraparound services.
3. Assists in the planning and implementation of medical care and coordination of services with other patient care units, both within the hospital and through outside institutions and links families with community resources.
4. Care is established from the time of admission to the service through termination by utilizing the nursing process in such a manner as to maximize outcomes.
5. Additional duties include the development and implementation of materials necessary to facilitate patient care with varied care providers, patient and family education, staff and care provider education, development of patient and family educational materials.
6. Participation in research, basic supportive counseling, procurement of community resources for families, and assessment of changing patient needs throughout the course of care with appropriate referrals for consultation as needed.

Responsibilities:
The Financial Case Manager is responsible for a multifaceted role, providing comprehensive support to clients. This role involves collaborating with the treatment team to align aftercare plans and assisting clients with employer and legal concerns. Additionally, the Financial Case Manager works closely with Facility Leadership to offer timely financial assistance, addressing insurance inquiries and collecting Copays/Deductibles as per Admission Department directives for each admission.
Works effectively with the facility leadership team to ensure success of the facility by completing the following:
Core Job Duties:
1. Maintain open communication and collaboration with the multidisciplinary treatment team to address
aftercare and discharge planning needs.
2. Develop individualized aftercare appointments by collaborating with patients.
3. Ensure follow-up appointments are made within 7 days of discharge from our facility with a behavioral
health provider.
4. If a patient is on MAT, be sure the patient has an appointment with a MAT provider after discharge.
5. Coordinate directly with referents and payors to ensure continued care needs are met.
6. Collect Co-pays, Deductibles, and private pay fees within 24 hours of the client's admission.
7. Assist patients in staying focused on their treatment by addressing employer and legal needs.
8. Conduct weekly discharge needs assessments and develop individualized aftercare plans.
9. Be able to assist in validating patients' benefits and financial responsibility using online applications such as
InstaMed, Navient, and Availity, or other relevant applications.
10. Follow the outlined procedures for the "Collection of Patient Responsibility."
11. Demonstrate a thorough knowledge and understanding of collection policies and procedures, specifically
related to Co-pays, Insurance Deductibles, setting up payment plans, and upfront private pay payments for
patients.
12. Assist client with COBRA election while obtaining confirmation of payment.
13. Collaborate with the Admission Team to find solutions and collect owed funds.
14. Ensure clients sign the “Coordination of Benefits”, “Authorized Claims Representative”, “Payment
Plan/Agreement”, “Financial Checklist”, other necessary consents, and complete the first CMDP within 72
hours of admission.
15. Facilitate the completion of an "Advance Repayment Agreement" for any uncollectable Co-pays or
Deductibles.

Role Overview
The RevOps Specialist will work with sales and marketing leadership to support organizational goals that help drive new business, implement and streamline sales processes and system changes to enable the sales and marketing teams to be successful. In addition, this role will use data to understand and report on various aspects of the sales and marketing processes -lead management, opportunity/ pipeline management, forecasting and performance.
ESSENTIAL FUNCTIONS (Responsibilities)
Infuses the Patch Values into your work ethic, every day and every interaction.
Support Sales leadership with necessary data to define Fiscal Year Strategy & Planning including org structure, go to market strategy, KPIs and Sales processes needed for success.
Deliver operational excellence in tracking and reporting on lead management, pipeline management, forecasting, and related analytics and insights to Sales, Marketing and Company Leadership that measure key business drivers.
Work with Business Process Management team members to optimize and support technologies in support of sales and marketing organization productivity by supporting implementation and driving adoption of the technology stack needed for growth.
Partner with Sales and Marketing Leadership to deliver new hire training on Sales technologies in use and related processes.
Partner with internal business teams to ensure any assigned projects are delivered on time and with quality.
Support data governance/stewardship for Sales and how they interact with business partners (Marketing/Legal/Finance) including recommendations on process changes, Rules of Engagement (ROEs), and systems to measure execution.
Build and deliver sales analytics and proactive insights to Sales and Marketing leadership and Executives.
Support internal processes, such as monthly sales, pipeline, forecast and reviews.
Support and drive CRM (Salesforce) standards including good hygiene for the sales and marketing.
Other duties as assigned.
POSITION REQUIREMENTS (Qualifications)
3-5 years of sales/revenue operations experience in SaaS
Thorough understanding of sales processes and methodologies
Working knowledge of Salesforce and related technologies used to support a sales organization.
Demonstrated interpersonal skills including mentoring, coaching, and facilitation skills with the ability to interact with colleagues at all levels.
Organized, detail oriented, and able to work autonomously with minimal direction.
Strong analytical skills and a structured approach to solving unique business problems.
Ability to thrive in a fluid, fast-paced, and unpredictable environment.
Four-year college degree from an accredited institution.
Must be legally allowed to work in the U.S.

The innovative approach allows individuals to age with dignity in their community. In Wisconsin, the focus is on partnering with various organization to accelerate the creation of Adult Family Homes and Community Based Residential Facilities. Currently Homecare Hub partners with > 10% of the existing small homes in Milwaukee, and multiple health systems across the state.
Role Description
This is a hybrid role for a case manager & care coordination position at Homecare Hub. This individual will help with placement of patients into small homes, and as well oversee a cohort of patients and assuring their clinical healthcare and non-clinical needs are met.
This hybrid role is located in Wisconsin with occasional travel across the state. Most in person work will be local, and there will be a component of work from home as well.
Qualifications
Individual can be either a nurse or a social worker. Other related fields will be considered.
Experience in the healthcare or caregiving industry
Excellent communication, interpersonal, and leadership skills.
Bachelor's or Master's degree in Nursing or Social work
