Essential Duties and Responsibilities
Coordinates registration, attendance, and follow-through for YoloCares for Kids (YCKids) programs, and partners with other YoloCares staff to promote programs.
Provides short-term individual telephone, and in-office anticipatory and grief support to the children and teens of YoloCares patients and clients.
Participates in the review and amendment of on-going Plans of Care for individuals enrolled in YCKids individual programs.
Provides short-term, developmentally appropriate anticipatory loss and grief groups for YoloCares adult clients and the wider community.
Makes referrals to community resources as appropriate.
Consults with the Center Manager regarding current complex cases to assess client's needs and develop and implement support and/or follow a Plan of Care.
Documents all interventions according to YoloCares policies and procedures and meets required time frames for their completion.
Trains and provides on-going support to YCKids volunteers.
In consultation with the Center Manager, offers classes, workshops, and presentations on grief-related topics.

Requirements:
Must be a registered nurse in the state, with current licensure to practice professional nursing
A bachelor's degree in nursing from an accredited program by the National League for Nursing preferred. Equivalent leadership experience may be substituted for a degree.
California specific: Must have a bachelor's degree in nursing from an accredited program by the National League for Nursing preferred.
Must have one (1) year of management experience, hospice or related health care organization preferred.

* Assist in closing out patient's clinical record when discharged
* Enter DARs and log/track their submission, if needed when online systems are down or emergency situations
* Assist in accurately entering patient billing data
* Assist in accurately processing and billing Medicare, Medicaid, private payer and patient claims in accordance with payor requirements and organization policy
* Assist in the preparation of monthly billing and accounts receivable reports, alert appropriate management team members regarding late or missing documents for billing
* Establish and maintain positive working relationships with patients, payers, and other customers
* Maintain confidentiality of patient and organization information
* Maintain adequate inventory of medical supplies/items needed to provide appropriate patient care
Requirements:
* Must have at least one (1) year of experience in health care data entry
* Preferred one (1) year of pervious health care related billing experience

Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children
Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record
Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services
Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations
Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification
Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols
Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care
Fulfill the obligation of requested and/or accepted assignments
Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
Complete all other duties as assigned
Requirements:
Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
Must have current nursing licensure in State
Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
Must have accreditation from an approved certifying body for advanced practice nursing as required by the State

Provides psychosocial support and counseling to patients, families and caregivers and identifies and coordinates community resource needs.
Responsibilities:
Ensures that patient/family psychosocial issues are addressed and that community resource needs are identified for each patient/family.
Performs and completes patient/family initial and on-going bio-psychosocial assessments consistent with YoloCares policies and procedures; ensure initial assessments are completed within five days of social work start date.
Contributes to the IDT in establishing and maintaining a Plan of Care for each patient/family.

* Assist in sending Doctor's orders to Physicians when received from nurses for his/her signature; keep current log of what is sent to make sure it is returned with signature

* Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
* Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record
* Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services
* Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations
* Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification
* Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
* Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners
* Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members
* Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician
* Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols
* Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care
* Fulfill the obligation of requested and/or accepted assignments
* Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues
* Complete all other duties as assigned
Requirements:
* Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
* Must have current nursing licensure in State
* Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
* Must have accreditation from an approved certifying body for advanced practice nursing as required by the State

Provide for funeral or memorial services for patients as requested
Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team

Actively participate in quality assessment performance improvement teams and activities

Requirements:
Must be a graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree
