The Medical Record Clerk's primary responsibility is to perform the medical record admission and discharge procedures for all inpatient, observation patients and emergency room patients. The clerk ensures accuracy, integrity and security of all databases. The clerk conducts database searches, processes all requests for medical record retrieval from permanent storage area/electronic medical records for internal and outside requesters. The clerk scans new and continuing records, for all patient encounters correctly into their electronic medical record. The clerk analyzes each patient encounter for deficiencies and notifies the physicians per Medical Staff Bylaws of incomplete charts. The clerk also obtains information needed in order to complete the birth certificate for all newborns and transmits information and files necessary reports to the Vital Records Department for the State of Louisiana. Job Setting and Description: This position is a vital part in the medical records department for this facility. The positon is full time, day shift. Normal working days and hours are Monday through Friday, 7:00 - 3:30 with a 30-minute lunch period. Adjustments to the normal schedule are required rarely and usually only under emergency circumstances.
