1. Actively cooperate with the Day Habilitation Director and other program staff in order to achieve the goals and objectives of the Day Habilitation Program.
2. Responsible for implementation of goal plans and data collection to monitor progress. Goal Plans are to be based upon the person's valued outcome as identified in their ISP.
3. Responsible for transporting individuals to volunteer and recreational activities in the Community. Also provide the necessary supervision, training and support during these outings.
4. Responsible for development of individual goal plans in accordance with the 3IP and 5 governing principles as assigned by the Direct Supervisor or Program Director.
5. Responsible for maintaining accurate daily participant records, charts, logs, and case/progress notes per the instruction of the Direct Supervisor and the individual treatment plan methodologies.

Essential Functions: Communicates and demonstrate a professional image/attitude for patients, families, clients, athletic directors, coaches, coworkers, and others. Documentation in athlete Electronic Medical Record to include health status, injury, illness, immediate care, treatment, rehabilitation and/or encounters. Document in Presagia EMR software the injury, treatment and encounter status of all student athletes for both home and away teams. Document at the beginning of each sports season all athletes' demographic information, team/sport participation and grade. All Injuries to athletes must be documented in Presagia within 24 hours. New injuries or illnesses should be created in the “Unresolved Injuries” or “Unresolved Illness” section and must include date and time of injury, body part, side, type of injury, severity, mechanism of injury, and injured while at. The injury must be linked to the appropriate team & sport at the time of injury. Initial management and recommendations as well as relevant history of the injury must also be thoroughly documented on the “Unresolved Injuries” or “Unresolved Illnesses” page when the injury is created. Initial assessment must be added as a note with note type “Initial Injury Assessment” and should document your thorough assessment of each injury or illness to include, history, observation, palpation, ROM, MMT, Neuro Eval, special tests, and differential diagnosis.
Provide return‐to‐play guidance to athletes, coaches and parents/guardians following injury under the supervision of Medical Director/ physician/provider. 7. Maintains appropriate level of communication with Medical Director and consults with Medical Director / provider regarding referrals for other services as needed. 8. Participating in continuing education/professional development activities. Attends/presents at departmental in‐service sessions as scheduled. Serve as a preceptor to graduate students in the clinical education portion of the Athletic Training Education Program.
Perform mandatory baseline concussion testing for incoming students in assigned sports.
Attend introductory team meetings and present athletic training information for assigned sports.
Participate in all pre-participation sport screenings and the medical clearance process.
Maintain accurate and up-to-date medical records on injured athletes. Obtain sports injury reports from injured athletes and forward to the insurance and referral coordinator for filing with the College athletic injury insurance plan.
Serve as consultant to coaching staff on conditioning, nutrition, and protective equipment; assist with prevention of injuries including education for athletes, identifying factors that put athletes "at risk", correcting deficiencies in athletes as deemed necessary, as well as taping and bracing.
Serve as liaison with parents, sports medicine physicians, health center staff, athletic trainers, and coaches in determining the best treatment options for the patient. Assist the team physician in determining if a player may return to competition.
Serve as host athletic trainer for league, regional and national championships competition held at Ithaca College.
Qualifications:
Master's degree in physical education, exercise science, athletic training or related field and a minimum of one year athletic training experience; possession of a valid BOC certification in athletic training and eligibility for New York state certification in athletic training; knowledge of medical terminology, knowledge of athletic training equipment and demonstrated ability to maintain confidentiality are required. Previous collegiate athletic training experience strongly preferred.
This position is 37.5 hours per week, 44 weeks per year.

Major Responsibilities
* Work within the team of medical staff to provide care for intercollegiate athletes including practice and contest coverage, prevention, evaluation, treatment, and rehabilitation of injuries and illnesses
