Case Management Provide treatment plan directed case management to assist individuals in accessing community services, supports, and housing. Participate in intra-agency and interagency case staffings for purposes of evaluation and treatment planning for individuals. Documentation Maintain accurate and timely clinical documentation in accordance with federal, state, county, and treatment program requirements under the direction of the clinical supervisor. Meet performance expectations for time and billing standards. Miscellaneous Work with a team of clinicians and supervisor for clinical supervision, treatment planning, staff and departmental meetings, and trainings. About your role: Clarvida is seeking a compassionate and motivated individual to join our Therapeutic Foster Care Team as a Family Care Certifier & Trainer (FCCT). In this role, you will be responsible for the initial certification and ongoing support, training, coaching, and retention of Family Care Providers (FCP) on your caseload, ensuring a high standard of care for youth in our Therapeutic Foster Care Program. Using your clinical expertise, you will assess prospective foster families to determine their caregiving capacity, focusing on creating a safe, stable, and nurturing environment. You will conduct home study evaluations, maintain comprehensive and up-to-date documentation, and coordinate both initial and annual training for Family Care Providers. As a key support for our FCPs, you will deliver ongoing training, build upon their caregiving strengths, and provide guidance during life transitions and challenges. Additionally, you will address performance concerns and ensure compliance with certification standards. Monthly, you will provide face-to-face support to FCPs, fostering strong relationships and promoting the highest quality of care. Skill training areas include activities of daily living (ADLs) and instrumental activities of daily living (IALDs), such as hygiene, medication management and support, grocery shopping, cooking, obtaining benefits and entitlements, scheduling appointments, transportation, social skills, development of individuals' natural supports, community integration, development of individuals' natural supports, etc. Prepare scheduled activities for ECS residents in conjunction with the QMHP. Case Management Provide treatment plan directed case management to assist individuals in accessing community services, supports, and housing. Case management includes making referrals and coordinating with other agencies to obtain services and resources for individuals; advocacy and brokerage on behalf of client to access community resources, benefits, and entitlements; and monitoring services.
