The Mental Health Professional will work as part of a multidisciplinary team with all other disciplines on the Behavioral Health Unit to provide a proactive and collaborative approach in serving the patient population.
MENTAL HEALTH PROFESSIONAL (LCSW, LCPC, LMSW, LPC) ESSENTIAL FUNCTIONS
Professionals will provide crisis stabilization, complete intakes and on-going evaluations, and provide consultation to in-house and community services.
Participate in monthly behavioral health calls, work collaborative with the medical team to best determine overall treatment for each individual.
The Behavioral Health Professional performs risk assessment of inmates; coordination of care with interdisciplinary team including medical staff, community providers, psychiatric hospitals and corrections; and discharge planning.
The Mental Health Professional assists in planning and implementing the goals and objectives of programs and projects.
The Mental Health Professional provides the necessary preparation of documentation, necessary records and reports.
The Mental Health Professional utilizes clinical skills such as prioritizing treatment needs; crisis management; brief clinical interventions including CBT and Motivational Interviewing.
Excellent and timely documentation skills required.
The ideal candidate is able to make independent clinical decisions, while utilizing assistance and supervisory intervention, as needed.

Job Summary
The Clinical Social Worker is responsible for assuming assigned patient caseloads on inpatient medicine and surgery, Addiction Recovery Program, or emergency department. Reporting to the Social Work Manager or a designated supervisor, the social worker will provide bio-psychosocial assessments and interventions for patients and/or families impacted by a patient's acute medical needs and any psychosocial factors or barriers impacting their care. The social worker will identify high risk psychosocial factors impacting a patient's health status and, as an integral member of the interdisciplinary team, will help the hospital staff understand the influence of those factors on the patient's course of medical care. The social worker will effectively intervene through evaluation, emotional supportive therapy and/or other therapeutic modality, provision of resources and recommendations, and team collaboration around potential barriers to care in the community and around discharge needs. The social worker will have knowledge in mandated reporting, cultural and/or age specific impact, homelessness, substance use disorders, psychiatric illness, and the various community agency and resources available. About the role
Job Summary: The Clinician I will assist the Director of Outpatient Services ensuring the provision of quality care in an outpatient mental health clinic serving individuals with co-occurring disorders. S/he will provide weekly supervision to Master Level clinicians, interns, or other members of the interdisciplinary team to ensure clients receive comprehensive assessments and individualized service plans; and provide group treatment services using evidence-based practices, as needed. They will also ensure service utilization benchmarks and reporting requirements for funders are met in a timely manner. As part of the Casa Esperanza, Inc. team, the Clinician I will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach, to ensure the coordination of all client services, both across Casa programs as well as with external providers. The Clinician I will support with creating, implementing and delivery trauma informed care across Casa programs. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessarily follow up and assignments; and ensure-wrap around support services are in place to maximize treatment effectiveness and clinical outcomes.
What you'll do
Clinical Services
Maintain an up-to-date waitlist and prioritize the assessment and appropriate placement of all high-risk individuals.
Monitor case assignments and ensure appropriate caseload mix.
Facilitate group interventions using evidence-based practices, as needed.
Provide support to clients through individual therapy
Utilizes case conferencing and the program team to coordinate and ensure client treatment plans are effective and appropriate.
Support with clinical group supervision to ensure effective communication among team members, manage productivity, and quality assurance.
Provide ongoing support as need for facilitation of the multidisciplinary team meeting and office-based addiction treatment meetings.
Educate clients and their families about psychiatric and substance use disorder diagnoses; recovery and wellness support services; medication including MAT for substance use disorders, psychotropic medications, and medications for other physical conditions; and ensure engagement with medication monitoring services and regular health maintenance
Build collaborative relationships within the community that support a strong recovery system of care.
Work with Director of Outpatient Services to ensure that all clinical staff receive training and regular performance evaluations that support the delivery of safe, effective, evidence-based, and culturally competent care.
Work with Director of Outpatient Services team to plan, implement, and maintain urgent care model.
Provide back up on-call coverage, as needed.
Complete all clinical documentation in the agency electronic record.
Provide SOAP coverage, as needed

Administrative
Work closely with the Director of Outpatient Services to ensure Casa Esperanza Outpatient facilities meet all licensing, regulatory, accreditation, credentialing and funder standards and requirements.
Maintains client and program records in accordance with agency, regulatory, and funder requirements and timelines
Work with Intake and admission Manager and Director of Outpatient Services to maintain efficient workflow to meet program utilization goals and objectives and ensure timely billing.
Work with Director of Outpatient Services to respond to internal and external data collection and reporting requirements.
Complete intern progress recordings, evaluations, and all required school documents
Participate in any required site visits and ensure compliance with any funder requirements.
Attend all relevant case conferences and staff meetings, including MDT.
Support with facilitation of MDT, OBAT, and FUOP staff meetings.
Provide weekly supervision to direct reports.
Attend weekly supervision
Attends all trainings and professional development opportunities as required.
On call on a rotation schedule
Ensure that all program components and evidence-based practices are delivered to the target population and address any barriers to implementation.
Collaborate with Director of recovery Support Services to oversee data collection and outcomes reporting, including progress on goals.
Support with creation, implementation, and delivery of trauma informed services in our integrated care model
Support with SOAP enrollment and tracking of completion

Qualifications
Master's degree in social work, psychology, counseling, or related field required
Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC)
Bilingual Spanish/English required
Two years' experience working individuals living with co-occurring addiction, mental illness and/or other trauma related disorders
Supervisory experience in a clinical setting and program management experience
Knowledge of appropriate community resources
Experience with homelessness, domestic violence, chronic disease, and incarceration
Experience in crisis management and intervention
Experience providing individual, family and group interventions to target population.

The Social Worker will ensure provision of appropriate, timely, and effective addiction recovery treatment for patients on a Level IV Detox Unit. Employs a range of clinical interventions such as individual, group, and family treatment. Collaborates with and provides social work consultation to other disciplines within the setting.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
-Conducts psychosocial assessment of patients to evaluate addiction/behavioral health history/emotional issues/coping style, understanding of illness/adjustment/compliance, barriers to care, cultural issues, abuse/neglect and domestic violence.
-Meets with patients daily to provide emotional support, aftercare planning, referrals for programs, and community resources. Identifies patients' psychosocial, financial, legal, dual diagnoses that effect patient care.
-When indicated, provides brief family systems work to provide psychoeducation, emotional support, and guidance around aftercare planning. This includes coordinating family/team meetings as needed, providing emotional support and community resources to patient/family management.
-Facilitates hour-long group sessions on various evidence-based topics, including Cognitive Behavioral Therapy (CBT), behavior change, and relapse prevention.
-Provides mandated assessments when abuse is suspected (child, disabled adult, elder) and safety assessment when domestic violence is reported. Files reports as indicated.
-Works effectively as part of the interdisciplinary health care team, communicating regularly with the team and other members on cases and as issues arise. Documents timely and relevant information.
-All work is documented in the patient's electronic medical record in a timely manner.
-Maintains high level of confidentiality for all patients in compliance with HIPAA and Federal guidelines.
-Facilitates the appropriate and efficient use of hospital and community resources.
-Participates in formal and informal clinical case reviews, clinical supervision, and educational seminars and other tasks as assigned by direct supervisor or Social Work Manager.

Qualifications
Master's Degree in Social Work from an accredited program.
Current professional licensure in Massachusetts (LCSW required, LICSW preferred).
Minimum of one year experience working with substance use disorders and effective treatment planning and discharge planning.
Internship/field placements in substance use treatment programs considered.
