Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan post-discharge (e.g., home exercise programs; recommending and / or providing equipment; recommending outpatient follow-up programs).
Participate in weekly case conferences and effectively communicate about patient care across disciplines involved.
Qualifications
Minimum of 1 year of physical therapy experience with a preference for 1+ years in an acute or a post-acute care setting.

The Pharmacist will be responsible for providing a strong customer service experience and clinical services to patients. The Pharmacist will greet patients and help with products and services that the Pharmacy provides. The Pharmacist will ensure patients are aware of the healthcare services provided including patient consultation, medication management or drug therapy reviews. Clinical or wellness services could include immunizations, diagnostic testing, and patient outcome services. The Pharmacist must have strong time management and organization skills to ensure they're scheduling their days around immunization appointments and daily walk ins while managing their other responsibilities.

Skills and Requirements
-BS in Pharmacy or Pharmacist Degree from an accredited education institution
-Current Licensed Pharmacist within the state of work
-Certified Immunizer APHA (this is in addition to license)
-6 months+ of Pharmacist working experience.
-Passionate about the industry, customer service/patient oriented and a drive to positively change the industry
* Open and interested to potentially convert FT as a Healthcare Services Pharmacist, Staff Pharmacist or Pharmacy Manager
-CPR or BLS Certification
-Hepatitis B Statement -- All Immunizers must complete a Hep B Virus Statement or declination.
-Willing to travel to multiple stores and long-term care facilities (dependent on location)
