What is this role about? Our older adult patients living in communities that we serve are faced with a variety of social challenges, called “Social Determinants of Health”, or SDOH. These challenges include a variety of difficulties that stem from lack of resources; and one of the examples is lack of transportation. At Gather Health, we have built a business model that allows these patients to have consistent access to quality healthcare, by creating a system by which we provide clinical care not only in our physical clinics, but also in their homes. Internally called “Medical Technician Partner”, this individual will provide in-home care and support for our patients and be the eyes, ears, and the hands of our medical teams in the home. They will work collaboratively with our primary care providers, Gather Guide Partners (who serve as a liaison between our patients and our clinics), and clinic administrators, as part of the care-team model. What are some of the day-to-day duties?
The day-in-the-life of an MTP will vary, depending on the needs of our patients. Some of the day-to-day duties will include but are not limited to:
Providing in-home care and support for our senior patients and serving as their clinical liaison with our primary care providers
Performing approximately six to eight home visits per day and addressing acute medical decompensation or complex transitions of care
Evaluate CHF and COPD exacerbations through physical examinations and assessing vital signs
Check in and organize an inpatient or ED admission avoidance plan
Conduct social visits with their patients in their homes or occasionally meet them in the ED
Make outbound calls to patients to check in regarding newly prescribed medications
Leverage and access medical control through the Clinical Medical Director as needed to deliver care
Document patient notes in the Electronic Health Record (Athena)

What are the requirements for this position?
Minimum of 1 year of experience working as a Medical Assistant, preferably in home health

Salary: $32,000 annually
Summary: Provides support for students within the building that may or may not be in a specialized education program.
Responsibilities:
* Accurately completes all required documentation and data collection daily.
* Works with an individual student to reinforce learning of material or skills as described in the IEP/treatment plan.
* Assists the teacher in devising special strategies for reinforcing material or skills based on an understanding of the individual student, his/her needs, interests, and abilities.
* Assists the student throughout the school environment as necessary.
* Alerts the regular teacher to any problem or special information about the individual student.
* Maintains the same high level of ethical behavior and confidentiality of information about the student as is expected of fully licensed teachers.
* Participates in in-service training programs as assigned.
* Responsible for other general duties as indicated by the Director of Special Education, Superintendent/Building Principal or his/her designee, and policies of the Haverhill School Committee

ESSENTIAL FUNCTIONS:
Performs CT procedures at a technical level not requiring constant supervision of technical detail.
Obtains patient history, explains standard procedures and addresses patient concerns.
Produces computerized tomographic scanner radiographs of specific areas as required by the departmental procedures.
Performs CT procedures at a technical level not requiring constant supervision of technical detail.
Performs those duties directly involved with a variety of technical procedures applying ionizing radiation for the purpose of detecting pathology.
Regulates the equipment used to expose the x-ray film, develops and documents PACS imaging.
Operates equipment safely and maintains SimonMed standards while performing call types of procedures.
Establishes and maintains a good rapport and professional relationship with fellow employees, other departments, and facility staff.
Familiar with standard concepts, practices and procedures.
Relies on experience and judgment to plan and accomplish goals.
Works under general supervision.

MINIMUM QUALIFICATIONS:
Requires registration as a Certified Radiologic and Computerized Tomography Tech by the State
Requires registration as an RT and CT Technologist by the A.R.R.T.
SimonMed Imaging requires valid hands on CPR certification
PHYSICAL DEMANDS: This position may require duties including lifting and carrying up to 40 pounds, sitting for prolonged periods of time, with frequent standing and walking.
