Responsible for evaluating the work performance of assigned staff and completion of their performance appraisal (PAS) within the required timeframe. Each family will be assigned a case manager who will address the range of family system issues that place children at risk of child abuse and neglect. They may also be assigned a skill builder who will work with the family to increase skills such as, but not limited to, parenting skills, co-parenting skills, problem solving skills, and stress management skills. The Social Services Assistant will provide outreach services to families in the Voluntary Case Management Program and provides hands-on, individual or group, culturally appropriate sessions that may include parenting, practical life skills, role modeling, nutrition, and planning. Additionally, they will provide administrative and clerical support to the management and staff of the VCM Program and provide back-up support to the Administrative Assistant. This position is under the direct supervision of the Program Supervisor. What you will be doing: Provides home visits or community-based visits to families referred to the program, focusing on parenting, problem solving, and communication skills and assisting the client in reducing risk factors and increasing protective factors. Perform the usual clerical duties such as word processing including report reporting, correspondence, telephone and in-person reception, filing, data entry, record maintenance, faxing, and timely and accurate data entry into PACT's eHana and/or assigned program's data system. Maintains confidentiality and tracks family files and family related information such as immunization records and medical home; responds to calls from referred clients explains the scope of service and/or the waitlist process; calls clients to cancel visits when necessary; and documents client contact in client records. Maintains client files from intake to discharge, including the insertion of external documents; providing information to funders as requested; and ensuring completeness of the client file at the end of service. Responsible for implementing parenting curriculum, knowledge of community resources available and to do warm hand-offs to service provider and understand how substance abuse and domestic violence affect families. Responsible for supporting families in completing goals on the IPP to reach permanency. Conducts assessments and observation forms in accordance with funder and state guidelines, and PACT's policies and procedures.
