Responsibilities
Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physician's orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.
Qualifications
Must be a graduate of an approved school of clinical education.
Must be licensed in the state where they currently practice.
Must have at least one year experience as a licensed professional.
Must have basic demonstrated technology skills, including operation of a mobile device.
Education and experience, preferred
Experience as a licensed professional may be deferred with a review and approval from the regional and executive service line clinical leadership.
Medicare home health or hospice experience is preferred.

Must possess a valid state driver license
Must maintain automobile liability insurance as required by law

REGISTERED NURSE MINIMUM REQUIREMENTS:
Graduate of an accredited School of Nursing
Licensure as a Registered Nurse in the state of employment.
Possesses an active CPR certification.
Remains knowledgeable about specific state laws and regulations governing practice.
Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.

Maintain accurate, detailed reports and records.
Administer medications to patients and monitor patients for reactions and side effects.
Record patients' medical information and vital signs.
Monitor, record, and report symptoms or changes in patients' condition.
Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
Modify patient treatment plans as indicated by patients' responses and conditions.
Monitor all aspects of patient care, including diet and physical activity.

LICENSED PRACTICAL NURSE MINIMUM REQUIREMENTS:
Graduate from a Licensed Practical Nursing program.
Currently licensed as a Practical Nurse in the state of employment.
Possesses an active CPR certification.

Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.
LICENSED PRACTICAL NURSE ESSENTIAL FUNCTIONS:
Utilizes a systematic approach to meet the health needs of each individual patient.
Implements nursing care within the LPN's scope of practice. (Includes compliance with all laws as applicable in the practice
setting).
Assists in the development and implementation of teaching plans based on the individual needs of the patient. The plans should speak to health promotion, maintenance, and restoration of health.
Cares for wounds with appropriate cleaning and dressing/bandaging.
Administers medications to offenders.
Provides for the care of multiple patients as directed by the Medical Director, DON, or Registered Nurse.
Monitors vital signs and reports changes to appropriate medical staff.
Documents actions in the MAR and medical records.
Other nursing duties as assigned by facility

Responsibilities
Provide coordination of care, ensure continuous assessment of each patient's and family's needs, and implement the interdisciplinary care plan. Perform services in accordance with the physicians orders and the established plan of care (POC). Ensure delivery of care is compliant with the federal and state regulations.
Qualifications
Must be a graduate of an approved school of clinical education.
Must be licensed in the state where they currently practice.
Must have at least one year experience as a licensed professional.
Must have basic demonstrated technology skills, including operation of a mobile device.
Education and experience, preferred
Experience as a licensed professional may be deferred with a review and approval from the regional and executive service line clinical leadership.
