The Agency has embraced the concept of a fully integrated system of care for all consumers of service. MAJOR FUNCTIONS:

The ideal candidate will have at least 1 year of experience as an LMSW and a strong background in discharge planning within a hospital setting. This is a temp-to-perm opportunity, and we are looking for local candidates to ensure immediate availability.
Responsibilities:
Conduct assessments and provide mental health support to patients.
Develop and implement care plans based on patient needs.
Manage discharge planning, coordinating services and resources for patients post-discharge.
Work collaboratively with healthcare teams to ensure the best patient outcomes.
Provide crisis intervention and support as necessary.
Qualifications:
Valid Licensed Master of Social Work (LMSW) license issued by NYSED.
Minimum 1 year of LMSW experience in a hospital or healthcare setting.
Experience with discharge planning and coordinating care post-hospitalization.
Recent hospital experience is strongly preferred.
Strong interpersonal and communication skills.

Work scope includes using evidence based, solution focused, behavioral oriented & skill building in which you will be trained and certified to counsel seniors who are challenged by depression, social isolation and anxiety. Treatment modalities will include both individual and group therapies in a supportive and collaborative environment. Strong computer skills a must.
