Our compassionate team provides a wide range of inpatient and outpatient services, including acute care rehabilitation, joint replacement & spinal surgery, neurosurgery, ICU, Telemetry, step-down care, skilled nursing, as well as outpatient therapy, hand and lymphedema clinics.
GENERAL DUTIES:
The individual in this position is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patient's resources and right to self-determination. The individual in this position has overall responsibility to assess the patient for transition needs including identifying and assessing patients at risk for readmission. Conducts complex psycho-social assessment and interventions to promote timely throughput, safe discharge and prevent avoidable readmissions. This position integrates national standards for case management scope of services including:
Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction
Care Coordination by demonstrating throughput efficiency while assuring care is sequenced and provided at the appropriate level of care
Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy
Education provided to physicians, patients, families and caregivers
Precepts new staff members and acts as resource to all staff.
Participates in department Quality Improvement initiatives, one committee participation and/or major projects as assigned
This individual's responsibility will include the following activities: a) complex psycho-social transition planning assessment and reassessment and intervention, b) assistance with adoptions, abuse and neglect cases, including assessment, intervention and referral as appropriate to local, state and /or federal agencies, c) care coordination, d) implementation or oversight of implementation of the transition plan, e) leading and/or facilitating multi-disciplinary patient care conferences including Complex Case Review, f) making appropriate referrals to other departments, g ) communicating with patients and families about the plan of care, h) collaborating with physicians, office staff and ancillary departments, i) assuring patient education is completed to support post-acute needs , j) timely complete and concise documentation in Case Management documentation system, k ) maintenance of accurate patient demographic and insurance information, l) and other duties as assigned.
QUALIFICATIONS:
Required qualifications include LCSW or LMSW based on license requirements of the state in which the Tenet Hospital operates. Must be currently licensed, certified, or registered to practice profession as required by law or regulation in state of practice or policy. Active license for state(s) covered.
Preferred: Accredited Case Manager (ACM). Minimum of two years acute hospital experience preferred. Required skills include demonstrated organizational skills, excellent verbal and written communication skills, ability to lead and coordinate activities of a diverse group of people in a fast paced environment, critical thinking and problem solving skills and computer literacy.

Essential Job Duties:
Conduct Social Work assessments to determine the psychosocial needs, preferences, and goals of the participants.
Actively participate in IDT meetings to develop participant care plans based on assessment findings.
Deliver and document social work interventions as outlined in participants' care plans, including arranging necessary resources and services, providing counseling, and offering case management.
Collaborate with the primary care physician and other members of the care team to ensure smooth care transitions between settings.
Initiate, coordinate, and facilitate care conference meetings to enhance care coordination among care team members, participants, and their support network.
Provide discharge planning for participants disenrolling from the program.
Job Requirements:
Masters Degree in Social Work (MSW) required.
Minimum of one (1) year of experience with a frail or elderly population.
Previous experience coordinating and facilitating care conference meetings.
Preferred previous experience assisting individuals with behavioral health and substance abuse issues.

Under the direction of the Director of Soboba Tribal Family Services, the Tribal Social Worker will provide direct services to Tribal Members and their families. Services include, but are not limited to acting as a liaison for the family with Child Protective Services; meeting with Tribal Members to assess their needs and help in resolving immediate/crisis issues that involve child abuse, neglect, substance abuse, domestic violence, parenting, anger management, trauma, grief/loss, mental health concerns, and elder abuse by providing information about and/or referrals to community resources; assist Tribal Members in accessing those services; and providing investigative and intervention services. To provide inhouse services: individual, family, couples and group counseling.
DUTIES AND RESPONSIBILITIES:
Provide direct services to Soboba Tribal members. These services include, but are not limited to:
Work collaboratively with Child Protective Services (CPS) and Adult Protective Services (APS). Attend all necessary meetings such as Children Family Team Meetings (CFTM) & monthly face to face visits.
Attend court hearings, prepare reports and declarations for the court as needed.
Meet with Tribal Member(s) to assess their needs. Assist in resolving issues as it relates to child abuse, substance abuse, domestic violence, anger management and parenting.
Work with Tribal Member(s) and their families who are in crisis as it relates to significant mental health issues including hospitalizations, conservatorships, and long-term care.
Awareness of the symptoms/problems related to alcoholism and drug abuse and how to advise families on assistance/treatment.
Provide substance abuse referrals and work collaboratively with Indian Health Clinic and Soboba Human Resources.
Provide individual and group counseling services to Tribal Member(s) and their families as needed regarding Anger Management, Parenting and Psycho Educational Topics/Materials.
Maintain monthly contact with Tribal Member(s), child(ren) and all service providers as required & maintain appropriate case documentation.
Provide educational support by advocating for Tribal Member(s), attending Individual Education Plan (IEP) or 504 Plan.
Accompany/assist Tribal Member(s) to visits/appointment/services as needed; Assist in completing non-legal forms. Provide resources and referrals as necessary.
Coordinate services to Tribal Member(s) & family which include phone calls, completing forms, placement options and conduct home evaluations.
Work Collaboratively with Soboba Department of Public safety by performing welfare checks on Tribal Members when necessary.
Assists with any needs regarding Soboba Elders and participate in Elders program when needed. Participate in Soboba Community Events/Outreach as needed.
Awareness of/ability to understand and learn Federal legislation that pertains to or affects the Native Tribes, particularly Indian Child Welfare Act (ICWA).
Cultural sensitivity to the special needs of the Native American community, particularly with regard to child abuse, domestic violence, substance abuse, mental illness, and elder abuse.
Safety issues pertaining to home visits and the potential for, and acknowledgement of, dangerous situations.
Effective oral and written communication skills, excellent interpersonal relation skills and the ability to interact at all socio-economic levels and backgrounds effectively.
Demonstrate knowledge and ability to utilize Microsoft Office and operate computer, cell phone and assigned office equipment.

EDUCATION:
