Summary
* This position is responsible for identifying patient/person and family psychosocial barriers to health care and community resources to meet their basic needs impacting well-being. This includes: collaborating with individuals and their families using the CM process for assessing needs, developing plan of care, coordinating delivery of services, monitoring changes, and reassessing the person's needs on a regular basis. Works in collaboration with SIM RN CM and Community Caregiver and with community agencies and providers across the continuum of care; facilitating the coordinated utilization of resources for maximization of health outcomes, patient/person and family satisfaction and financial outcomes.

Duties
* Assesses advanced illness management patients considering patient and families values, needs and preferences to identify psychosocial barriers to health care resources and basic needs impacting their well-being.
* Actively participates with other members of the highly functional SIM team, with the person/patient at the center, to achieve genuine collaboration and facilitate superior health outcomes for these complex patients
* Partners with individuals/families/caregivers to Utilizes evidenced based practices and tools to support SIM model (patient engagement; self-management/health literate care; SBAR; motivational interviewing, teach back; etc.) to develop advance care plan and lifespan plan which promote informed, activated, and engaged self-management.
* Assesses plans, develops and facilitates appropriate services and resources through collaboration with physicians, pts and their families and the IDT to prevent unnecessary delays in care, Increased LOS, and avoidable readmissions.
* Provide emotional supportive intervention to patients and their families throughout the system.
* Completes documentation of assessments and interventions in the medical record and other records as required.
* Performs other duties as assigned.

An employee in this class provides counseling and guidance to assist consumers in their personal recovery process including addressing such clinical issues as developing crisis response plans, rolling with resistance, harm reduction, motivational interviewing, problem solving, social skills training, education and development of natural support. Duties include completing standardized assessments, developing and completing treatment plans in conjunction with mental health treatment providers, individual counseling, group counseling, individual/group education, facilitating completion of treatment plan goals, and subsequent daily documentation. All work is in accordance with the requirements of the Missouri Department of Mental Health Integrated Dual Diagnosis Standards.
