The after school social worker supports the social, emotional, behavioral, and academic growth of students. They recognize and address any social, emotional, and behavioral concerns that may get in the way of a student's academic progress. This position furthers the mission of Étoile Academy because it helps ensure that all students have a safe environment to learn during our after-school programming. Hours: 10:00 AM - 6:00 PM Responsibilities: The Stronger Connections After School Social Worker will promote student success, provide preventative services, and respond to identified student needs by implementing a comprehensive developmental school mental health program that seeks to meet the needs of the whole student. The After School Social Worker will seek to develop and facilitate programming that effectively removes barriers to learning and equips students with the necessary tools needed to navigate their educational experience and beyond. Étoile Academy Student Support Services exists to prevent, respond to, and minimize the barriers to learning that students may experience. Reporting to the Director of After School, the Stronger Connections After School Social Worker is responsible for developing, coordinating, and facilitating comprehensive mental health programming for after school. This may include providing direct support to students, coordinating resources for families, coordinating data-driven interventions and programming, and facilitating professional development for staff and parents. The Stronger Connections After School Social Worker will have responsibilities such as, but not limited to, the following: Selects and administers age-appropriate assessment methods and materials in order to determine the needs of the student. Develops and implements direct intervention strategies for students, teachers, and families to resolve stressors and remove barriers that interfere with student success. Conducts specialized individual and group counseling sessions to address specific emotional, social, and behavioral needs of students. Mobilizes and coordinates community resources for students and families to address academic, behavioral, emotional, and social needs. Develops and creates strategies for parents, teachers, and students on instructional and behavioral issues. Develops and builds community agency partnerships with service providers to expand resources available to students and families. Provides crisis management services, including assessing for safety. Advocates for student services and students' best interests. Provides case management services including, but not limited to, referrals to community resources, collaboration with other professionals. Provides training and workshops to teachers, school staff, and parents. Serves as a liaison between the student, home, school, private counseling facilities, and community resources such as social services, court services, and family service agencies. Tracks and analyzes data on student success through a variety of Étoile software platforms. Other duties as assigned to support the mission of Étoile and after-school programming. Qualifications: Education and Knowledge Accomplished professional experience in one or more of the following settings: PK-12 schools, early childhood centers, out-of-school-time programs. The ideal candidate will have the following experience, expertise, and skills: Master's degree in school psychology, social work, psychology, counseling, or related field. Must hold one of the following license types: LPC-I, LPC, LPC-S, LMSW, LCSW, or LMFT or TEA School Counselor Certification. Demonstrated facility with utilizing data to inform programmatic decisions. Intervenes with patients and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs. Under supervision this may include short term individual, marital and family therapies as well as crisis intervention. Provides intervention in cases involving child abuse/neglect, domestic violence, guardianship (temporary/ permanent), institutional abuse, foster care, adoption, mental health placement, advance directives, adult/elderly abuse, child protection and sexual assault. Serves as a resource person and provides counseling and intervention related to treatment decisions and end-of-life issues. Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system. Participates in discharge planning activities for complex patients, in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers. Deals with families exhibiting complex family dynamics that impact directly on patient care and discharge. Communicates with clinical care team members regarding the discharge planning status of all patients referred by them. Provides consultation to Case Managers when coordination with significant or intensive community resources is necessary to achieve desired treatment outcomes. Receives referrals for complex patient problem resolution from Case Managers or clinical care team members. Works in collaboration with the clinical and case management team members on transition planning and referrals to post acute care providers. Keeps clinical and case management team members up-to-date on the status of the post-acute provider acceptance and clearance for discharge. Validates discharge criteria for patient and families and notifies clinical and case management team members of newly-identified resources or change in previously-identified resources. Educates patient/family and physician regarding post-acute options and addresses issues of choice. Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.
