5. Additional duties include the development and implementation of materials necessary to facilitate patient care with varied care providers, patient and family education, staff and care provider education, development of patient and family educational materials.
6. Participation in research, basic supportive counseling, procurement of community resources for families, and assessment of changing patient needs throughout the course of care with appropriate referrals for consultation as needed.

Responsibilities:
The Financial Case Manager is responsible for a multifaceted role, providing comprehensive support to clients. They ensure compliance with State and Federal guidelines, conduct weekly discharge needs assessments, and develop individualized aftercare plans, including making appointments for ongoing care and coordinating continued care needs with referents and payors. This role involves collaborating with the treatment team to align aftercare plans and assisting clients with employer and legal concerns. Additionally, the Financial Case Manager works closely with Facility Leadership to offer timely financial assistance, addressing insurance inquiries and collecting Copays/Deductibles as per Admission Department directives for each admission.
Works effectively with the facility leadership team to ensure success of the facility by completing the following:
Core Job Duties:
1. Maintain open communication and collaboration with the multidisciplinary treatment team to address
aftercare and discharge planning needs.
2. Develop individualized aftercare appointments by collaborating with patients.
3. Ensure follow-up appointments are made within 7 days of discharge from our facility with a behavioral
health provider.
4. If a patient is on MAT, be sure the patient has an appointment with a MAT provider after discharge.
5. Coordinate directly with referents and payors to ensure continued care needs are met.
6. Collect Co-pays, Deductibles, and private pay fees within 24 hours of the client's admission.
7. Assist patients in staying focused on their treatment by addressing employer and legal needs.
8. Conduct weekly discharge needs assessments and develop individualized aftercare plans.
9. Be able to assist in validating patients' benefits and financial responsibility using online applications such as
InstaMed, Navient, and Availity, or other relevant applications.
10. Follow the outlined procedures for the "Collection of Patient Responsibility."
11. Demonstrate a thorough knowledge and understanding of collection policies and procedures, specifically
related to Co-pays, Insurance Deductibles, setting up payment plans, and upfront private pay payments for
patients.
12. Assist client with COBRA election while obtaining confirmation of payment.
13. Collaborate with the Admission Team to find solutions and collect owed funds.
14. Ensure clients sign the “Coordination of Benefits”, “Authorized Claims Representative”, “Payment
Plan/Agreement”, “Financial Checklist”, other necessary consents, and complete the first CMDP within 72
hours of admission.
15. Facilitate the completion of an "Advance Repayment Agreement" for any uncollectable Co-pays or
Deductibles.

Relationship Management
Builds trusted relationships with patients, prescribers, and appropriate client stakeholders regarding reimbursement inquiries and challenges through proactive communication, timely and accurate execution of deliverables and demonstrated relentless passion for helping patients.

Managed through call/contact center structure, this role supports inbound and outbound calls to patients, caregivers, specialty pharmacies and healthcare professionals.

Identifies and communicates patient's plan benefit coverage including the need for prior authorization, appeal, tier exception, and/or formulary exclusions.
Serves as a subject matter expert to internal team as required and appropriate.
Uses electronic resources to obtain benefit coverage outcome and if needed, outbound call to payers and HCPs to follow up on proper submission and/or outcome.
Coordinates nurse teach with nurse educators, as applicable to program
Supports adherence services as applicable to program.
Identifies peer support resources for patients.
Proactively communicates needs for reverification of prior authorization or re-enrollment.
Identifies and reports adverse events, product complaints, special situation reports and/or medical inquiries received in accordance with program operating procedures and the Business Rules

Requirements:
Completion of Bachelor's degree (or higher) required. Degree in healthcare administration, social science or similar related fields is strongly preferred. In lieu of a degree, five plus years of experience in insurance reimbursement, patient access, direct patient care, and/or patient education is required.
Two years of experience in insurance reimbursement, patient access, direct patient care, and/or patient education is required.
Minimum two years of experience in healthcare access delivery or management is strongly preferred.

Job duties:

· Complete face-to-face CPST assessments within (5) business days after receiving referral.
· Maintain caseload list reflective of current and active clients. Review closure / discharge if no services within 90 days.
· Two year degree in a relevant social science field or equivalent education and experience
· Minimum of two (2) years' experience in the mental health and/or drug and alcohol field.
· Working knowledge of mental health, drug and alcohol, and other community agencies and resources essential.

The Bilingual Collaborative Care Clinician I is an associate-level licensed behavioral health clinician reporting to the Clinical Excellence Manager. This role provides evidence-based interventions via the Collaborative Care Model to individuals (pediatric and/or adult populations) experiencing mild to moderate symptoms and utilizes evidence-based therapeutic interventions to promote symptom reduction and optimal patient outcomes.
