MHS is hiring married couples to become flex houseparents. Flex houseparents are couples who reside in on-campus student homes, guiding and nurturing a group of approximately 8-12 students. Flex houseparents provide a consistent family-like structure for students and handle responsibilities such as driving, administering medication, budgeting, reporting, etc. When initially hired, flex houseparents cover different student homes before being assigned to one specific home.

The Bereavement Care Provider will provide telephonic and in-person crisis intervention, grief counseling, and other support as needed primarily to individuals affected by drug-related deaths. This position requires a high level of clinical responsibility, integrity, and initiative.

Child and Youth Counseling
Offer trauma-informed counseling to clients whose loved ones have died from an accidental drug intoxication or who have experienced trauma related to a loved one's substance use;
Mitigate barriers to transportation for clients by providing in-home or in-school counseling to a percentage of your caseload and be able to communicate and schedule these sessions with schools, caregivers (and Free Library or other community locations when needed).
After performing initial outreach and assessment, work with child and family to develop age-appropriate goals and treatment plan. Adjust as necessary over treatment period and keep family informed of progress.
Perform periodic calls to caregivers to provide emotional support, psychoeducation about developmental grief, and assess for positive or negative changes in child, as perceived by the caregiver;
Adult Counseling
Manage a caseload of adult clients seeking short-term counseling for people who lost a loved one from a drug-related loss.
Start up and facilitate at least one peer-support group for grieving children and adolescents and/or adults who have suffered a drug-related death;
Develop and facilitate at least one psychoeducational workshop on a topic related to grief following the loss of a loved one to substance use;
Case Management/Resources/Referrals
Develop and nurture connections with internal and external providers (such as school admin and counselors, outpatient community mental health agencies, etc.) to support effective treatment for clients.
Offer clinical insight and support to continue to build a program that provides effective, accessible services that accurately meet the needs of the Philadelphia community.
Assist other team members with community events or projects as needed.
Enter all case notes into the database in a timely fashion and complete all assigned tasks; including related duties not specified here.

Constellation Health Services is seeking a Home Health Aide (HHA) to join our Home Care Team. The Home Health Aide (HHA) is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse or licensed therapist and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The specific personal care for each patient will be determined by the registered nurse or therapist and will be given in writing to the certified home health aide - HHA. The HHA is responsible for observing the patient, reporting these observations and documenting observations and care performed. The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care.
Responsibilities:
Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality.
Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 ,etc.)
Giving personal care including baths, (sponge, tub or shower) back rubs, care of hair, skin, oral hygiene, shampoos if directed by the registered nurse.
Cleaning of nails and use of emery board. No cutting or clipping of nails is permitted.
Performs light housekeeping duties necessary to maintain a safe environment including changing bed linen, light cleaning of patient area, and laundering essential to the comfort and cleanliness of the patient.
Assisting in dressing and undressing patients as assigned.
Planning and preparing nutritious meals, including shopping, as assigned.
Assisting in feeding the patient as assigned.
Assessing and reporting on patient's condition and significant changes to the Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues.

Requirements:
Certified Home Health Aide in the state of employment.
Meets the training requirements of the State, including ten hours of orientation and a competency evaluation prior to providing patient care.
