Patient-Centered Care

We Are There Wherever You Need Us
We provide care for patients wherever they call “home” - private residences, assisted living facilities, long-term care facilities, hospitals, group homes and veteran's facilities. Patients who need acute symptom management can be admitted to our 20-bed Kaplan Family Hospice House, which opened in 2005 as the first licensed inpatient hospice facility in Massachusetts.
Palliative Care for Advanced Illness
For patients who aren't ready for hospice, our palliative care program adds a layer of support for patients with serious illness by providing relief from pain and other symptoms; assisting families in making difficult medical decisions; coordinating care with the patient's physicians; and providing emotional and spiritual support.
Reaching Out to the Community with Education
The staff at Care Dimensions provides outstanding training and accredited continuing education opportunities to healthcare professionals. We also offer workshops and classes to members of the community who seek to better understand issues surrounding advanced illnesses and end-of-life decisions.
Helping Families Cope with Loss
Care Dimensions is widely recognized for its bereavement program - considered to be the most comprehensive in New England. We help honor the memory of those lost by providing grief support to 5,000 adults and children each year. We offer counseling services, workshops, support groups, bereavement camp, and crisis intervention at schools, businesses and community organizations.
