- Partners with community team to ensure community is in compliance with OSHA requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.
- Responds to the dining needs of the residents and guests while maintaining Sunrise hospitality and service standards
- Invite, Encourage, Assist (IEA) residents in life skills and other life enriching activities as indicated on ISP and demographic profile

Qualifications:

Advance Practice Provider Transition into Practice Fellowship
Ann and Robert H. Lurie Children's Hospital of Chicago offers an Advance Practice Provider (APP) transition into practice fellowship for all new graduate APPs. All new hire APPs, even those with experience, are invited to participate in the fellowship. The APP transition into practice fellowship is designed to provide a supportive structure and strategies to facilitate the transition from the academic to clinical setting for newly hired APPs and institution to institution transition for those with experience.
The first year of employment will consist of quarterly didactic sessions with integrated facilitator led peer supported transition sessions, structured mentorship, and precepted practice-based learning with standardized competencies. The Advanced Practitioner - Organ Donation is an advanced practitioner with demonstrated leadership skills and clinical excellence. The individual functions as a healthcare provider who, working collaboratively with the various teams at RUSH University Medical Center and Gift of Hope, is responsible for providing comprehensive care to potential organ donors transferred to the Donor Care Unit with the goal of maximizing organ utilization. They demonstrate a high degree of clinical expertise in working with patients with acute and chronic illnesses commonly encountered in the brain dead and neurologically injured population. The APP - Organ Donation will provide both donor management care as well as intraoperative care to the donor throughout the entire length of stay.
The advanced practice position demonstrates an advanced level of medical/nursing clinical knowledge, communication and interdisciplinary collaboration, professionalism, and systems-based practice. They work with administrative, nursing, physician, and collaborating clinical services to assure safe, effective quality patient care and to promote the Donor Care Center's strategic goals.
The advanced practitioner will provide care within their scope of practice as outlined by applicable state law, licensing, regulations, institutional policy, and practice agreements. In collaboration with the Medical and Executive Director of the Donor Care Unit, the individual will lead the development, implementation, and evaluation of donor organ utilization to ensure donors are managed to optimize organ viability and function and maximize gifts to save more lives and optimize organs transplanted per donor.
The individual will utilize evidence-based practice implementation experience and work collaboratively with transplant centers and OPOs nationally to improve the donor management processes. The individual will be trained by the Medical Director and Executive Director on organ donor medication delivery practices and will escalate clinical concerns to the Medical Director and Executive Director as needed.
The individual will develop and maintain essential relationships with medical and administrative leaders at transplant centers and focus on improving the donation and transplantation of organs with Gift of Hope staff and leadership to improve the number and quality of organs.
The Advanced Practitioner will lead and participate in policy development, quality improvement initiatives, and training of donor management strategies while maintaining regulatory compliance and monitoring outcome measures. The individual works collaboratively with the Executive Director, Medical Director, department managers and others to provide advanced clinical practice expertise, implement evidence-based practices, oversee and improve clinical donation practices.

- Scheduled service hours cannot be changed nor modified without the authorization from the direct supervisor
- When providing in-home services strictly follows a participant/client's written Plan of Care;
- Reports to the Supervisor with any temporary changes or deviations from the Plan of Care or schedule;
- Carries out duties as assigned by the Supervisor.
- Performs routine housekeeping tasks, such as making and changing beds; dusting; washing dishes; vacuuming; keeping the kitchen and bathroom clean; doing laundry.
- Provides hands on physical assistance for toileting, bathing and other related activities of daily living (ADL.)
- Plans, shops for, and prepares nutritious meals, or assists participant/client in planning, shopping for, and preparing nutritious meals; assists with meals, including serving meals and feeding.
- Assists with written special diet plan following and reinforces maintenance of the diet.
- Assists with transporting and transferring participants as needed.
- Assists, teaches, and/or performs patient clothing care and assists client during the physical therapy under the supervision of licensed medical personnel.
- Treats clients/employees and their families with respect and dignity.
- Observes participant/client's functioning and health condition, and reports it to Supervisor; reports any status changes such as client phone number.
- Provides necessary receipts and documentation in case of essential shopping/errands, and fills out “Two-Way Receipt.”
- MUST use electronic clock in and out system correctly and on time, using client's home/landline phone.
- MUST report to Supervisor as soon as possible any changes in schedule.
- MUST notify supervisor immediately if you forget to clock in or clock out, and MUST submit timesheet with Client's signature (original) as soon as possible but no later than 5:00 PM the payroll scheduled due date.
- MUST submit a Monthly Homemaker Service Report with the client's original signature no later than 10th of following month.
- Calls 911 in case of emergencies and then contacts direct Supervisor.
- Reports to the Supervisor as soon as possible with any absences and/or coming late, but no later than two hours before the regularly scheduled start time.
- Protects own health and health of others by adopting safe work practices, reporting unsafe conditions immediately, and attending all relevant in-services regarding occupational health and safety.
- Maintains professional, friendly, courteous, caring relationship/atmosphere with all staff members.
- Works in team to assure accomplishment of the company's goals.
- HCA IS NOT ALLOWED TO PERFORM MEDICAL TASKS, such as:
Administer shots, including insulin.
Pour any medication or place medication in the patient's mouth.
Administer any enema.
Administer eye drops.
Change the dressing on a wound.
Cut the patient's finger-or-toe nails.
Administer prescription lotions.

Regulatory Requirements:
Must be at least eighteen (18) years of age.
Must have legal authorization to work in the United States of America.
High school diploma or GED equivalent is required or one year of documented supervised work experience
in the community care program (CCP) or one year of employment in a comparable human service capacity, or experience in care for a dependent child or adult family member.
Must complete the EUROPEAN SERVICE LLC pre-service training program and achieve a score of 80% or greater; or provide evidence of 40 hours of prior supervised training as a home care aide/caregiver within two years.
Must submit to and pass a pre-employment background check.
Prior experience in working with the elderly population preferred.
Language Skills:
Able to communicate effectively in English, both verbally and in writing.
Additional languages preferred.
Skills:
Excellent human relations skills with the ability to communicate effectively and deal courteously with the participants/clients, their families, fellow employees, public on the telephone or in person even though they may be irate and unreasonable at times.
Knowledge of all aspects of In-Home Care.
Physical Demands:
Be in good physical health and provide TB test documentation.
Must possess physical and mental ability to work independently.
Work is typically performed standing and sitting; however, walking, bending, stooping, reaching and lifting objects weighing up to thirty (30) pounds is required on an intermittent basis.
Works indoors and outdoors. Occasional trips with participants/clients may be taken to locations outside their homes, such as to physicians' offices or on outings, using a motor vehicle.

PM Pediatric Care is hiring Advanced Practice Providers with pediatric experience to join our team. Under the guidance of the Regional Medical officer, APPs are responsible for evaluating and providing evidence-based treatment to pediatric patients in an urgent care setting.
Job Responsibilities:
Assess and treat acute injury and illness for both in-person and telemedicine patients; including but not limited to the management of the febrile infant/child, ear/nose/throat infections, pediatric abdominal pain, anaphylaxis, procedural anxiolysis, respiratory distress, croup, and asthma.
Perform procedural care including but not limited to abscess drainage, foreign body removal, and laceration repair with sutures, staples, and dermal adhesive.
Take radiographs and provide preliminary interpretation of x-rays with official radiology over-reads (only in applicable states). All necessary training will be provided.
Oversee in-person and telemedicine patient flow and daily functions of the office along with oversight of other providers, staff, and trainees on shift.
Maintain operations by following policies and procedures; reporting needed changes to the management team.
Work in conjunction with the onsite and telemedicine teams to assure completion of daily tasks including but not limited to lab and radiology result review, patient follow-up, and timely responses to patient calls and questions. Maintains safe, secure, equitable, and healthy work environment by following and enforcing safety procedures, quality standards, and complying with legal regulations.
Maintain patient confidence and protect operations by keeping patient care information confidential in compliance with HIPAA regulations.
