The Advanced Practice Provider (APP) provides care in the acute care inpatient setting based on educational preparation and certification in either the APN or PA role specific to the Vascular patient population. Key responsibilities include coordinating patient care within the Division of Vascular Surgery, collaborating with attending physicians, resident house staff, Advanced Practice Providers, and other multidisciplinary team members. As an expert practitioner, the APP designs individualized treatment plans, evaluates outcomes, and modifies the treatment plan as needed. The APP facilitates the transition of patients between service units and provides/coordinates complex patient and family teaching.
What you'll do:
Oversee the care of all floor and stepdown primary patients.
Respond to consult requests when the resident team is in the operating room or unavailable.
Collaborate with social work, hospitalist, and consulting teams to ensure coordinated patient care.
Facilitate patient education and assist with discharge planning processes.
Manage dressing changes and wound care, including coordinating home health services (e.g., physical therapy, occupational therapy, and wound care).
Organize and prepare discharge medications.
Coordinate post-discharge services, including assessment for acute inpatient rehabilitation or transfer to a nursing facility.

Performs other duties / functions as requested by supervisory personnel.
Minimum Requirements:
Education/Experience:
High school diploma or GED equivalent required; Bachelor s degree in human services preferred;
One year of child welfare experience working with children and/or adolescents
Two years experience working with youth in a residential setting preferred.
Skills Knowledge:
Bilingual (Spanish/English) language competency required;
Ability to use good judgment, and think quickly and rationally in stressful situations;
Successful completion of all staff education requirements including: Pre-Service training, NCI, CPR, and ARC Model;

PM Pediatric Care is hiring Advanced Practice Providers with pediatric experience to join our team. Under the guidance of the Regional Medical officer, APPs are responsible for evaluating and providing evidence-based treatment to pediatric patients in an urgent care setting.
Job Responsibilities:
Assess and treat acute injury and illness for both in-person and telemedicine patients; including but not limited to the management of the febrile infant/child, ear/nose/throat infections, pediatric abdominal pain, anaphylaxis, procedural anxiolysis, respiratory distress, croup, and asthma.
Perform procedural care including but not limited to abscess drainage, foreign body removal, and laceration repair with sutures, staples, and dermal adhesive.
Take radiographs and provide preliminary interpretation of x-rays with official radiology over-reads (only in applicable states). All necessary training will be provided.
Oversee in-person and telemedicine patient flow and daily functions of the office along with oversight of other providers, staff, and trainees on shift.
Maintain operations by following policies and procedures; reporting needed changes to the management team.
Work in conjunction with the onsite and telemedicine teams to assure completion of daily tasks including but not limited to lab and radiology result review, patient follow-up, and timely responses to patient calls and questions. Maintains safe, secure, equitable, and healthy work environment by following and enforcing safety procedures, quality standards, and complying with legal regulations.
Maintain patient confidence and protect operations by keeping patient care information confidential in compliance with HIPAA regulations.

Companion Caregiver Responsibilities
Help clients take prescribed medication
Assist clients with ambulation and mobility around the house or outside (doctor's appointments, walks, etc.)
Must be willing to assist clients with personal care and hygiene, including but not limited bathing assistance, brushing hair and teeth, or applying skincare
Help clients with routine exercises
Plan and prepare meals with assistance from the clients (when they are able)
Do the client's shopping or accompany them when they shop
Perform light housekeeping duties that clients can't complete on their own
Be a pleasant and supportive companion
Report any unusual incidents
Act quickly and responsibly in cases of emergency
Companion Caregiver Requirements
Passion to serve others - a true caregiver
Excellent knowledge of emergency response and first aid
Knowledge of housekeeping activities and cooking with attention to dietary constraints
Willingness to adhere to health and safety standards
Respectful and compassionate
Good time management skills
Outstanding communication and interpersonal skills
Strong ethics
Physical endurance
High school diploma or equivalent
