The Advanced Practice Provider (APP) provides care in the acute care inpatient setting based on educational preparation and certification in either the APN or PA role specific to the Vascular patient population. Key responsibilities include coordinating patient care within the Division of Vascular Surgery, collaborating with attending physicians, resident house staff, Advanced Practice Providers, and other multidisciplinary team members. As an expert practitioner, the APP designs individualized treatment plans, evaluates outcomes, and modifies the treatment plan as needed. The APP facilitates the transition of patients between service units and provides/coordinates complex patient and family teaching. What you'll do: Oversee the care of all floor and stepdown primary patients. Respond to consult requests when the resident team is in the operating room or unavailable. Collaborate with social work, hospitalist, and consulting teams to ensure coordinated patient care. Facilitate patient education and assist with discharge planning processes. Manage dressing changes and wound care, including coordinating home health services (e.g., physical therapy, occupational therapy, and wound care). Organize and prepare discharge medications. Coordinate post-discharge services, including assessment for acute inpatient rehabilitation or transfer to a nursing facility.
