Advanced Practice Provider, Geriatric Care Clinic - (847982) Description Why UT Southwestern? Job Summary The Advanced Practice Provider (APP) collaborates with a multidisciplinary team within the UT Southwestern Geriatric Care Clinic to provide comprehensive primary care to geriatric patients with unique and, often, multiple health concerns. This position will require a high degree of critical thinking and clinical expertise in working with outpatient older adults. The Nurse Practitioner or Physician Assistant is responsible for assessment, diagnosis, treatment, management, education, consultation, health promotion and care coordination for patients and families. They develop care plans that address the special health care needs of older adults, conduct and document comprehensive physical examinations based on age and diagnosis, and evaluate urgent medical needs. Experience and Education Required Education Master's degree or higher in nursing, documentation of completion of accredited Advanced Practice Education Program. Graduate education and related Texas Board of Nursing authorization must be consistent with clinical practice area and patient population.
