The ideal candidate is someone who passionately cares for providing high quality care to patients and enjoys the practice of medicine, while understanding the importance of having great bedside manners.
Candidates should have at least one year of experience in Urgent Care, Family Medicine or Emergency Medicine.
Candidates will be expected to be able to work 12-hour shifts, weekends, and major holidays.
DOT Certification will be required as we provide services for Occ Med Patients.
Worker's Compensation experience is preferred.

Job Summary:
The goal of the Advanced Practice Provider (APP) Clinical Gastroenterology Fellowship is to provide a 1-year postgraduate gastroenterology training program for an APP in a clinical outpatient setting, designed to develop a specialization in gastroenterology through didactic education and clinical experience. The curriculum will provide the fellow with the background information necessary to successfully identify the main clinical manifestations, causes, and the differential diagnosis of the major types of gastrointestinal diseases and their complications.
The curriculum will be primarily focused in our outpatient GI setting with core rotations including general digestive health, therapeutics (pancreas and hepatobiliary disease), esophageal clinic, inflammatory bowel disease and motility disorders. In addition, the fellow will have exposure within our subspecialties including hepatology, colorectal surgery, nutrition, hereditary colorectal neoplastic clinic, and inpatient hospital service. The core clinic rotations will range from four to eight weeks and the subspecialty rotations will range from one to three weeks.
In between clinic days, the fellow will have 1 day per week for didactics which may be done on campus or at home. During this time, the fellow will be responsible for completing assigned reading materials and modules through the American College of Gastroenterology Education Universe Portal. This membership cost will be covered by the CME budget.
At the end of each rotation, there will be a competency multiple choice exam as well as an assessment completed by the fellow and preceptor. There will be biweekly meetings with the Program Director via zoom.
Completion:
Upon graduating, the fellow will receive a certificate from Colorado University School of Medicine which verifies that they have completed all components outlined in the curriculum and have achieved program competencies.

The Colorado Mental Health Hospital in Pueblo (CMHHIP) is a 516-bed acute care psychiatric hospital that provides inpatient behavioral health services for adults, adolescents and geriatric patients. CMHHIP is a forensic hospital that serves individuals with pending criminal charges who require evaluations of competency, individuals who have been found to be incompetent to proceed (restoration treatment) and individuals found to be not guilty by reason of insanity by Colorado's criminal courts.
The Advanced Practice Provider (APP) diagnoses and treats patients with mental, emotional, and behavioral disorders under the general supervision of or in collaboration with a staff psychiatrist. Psychiatric providers employed by the University of Colorado to provide care to patient in CMHHIP shall be responsible for offering professional psychiatric services to patients in compliance with CMHHIP Clinical Services and mental health guidelines, to include psychiatric assessment of patient's mental health needs, prescription and management of psychiatric medicine, clinical management of patients with mental illness, consultation with other staff regarding the mental health needs of patients, provision of training to ancillary staff regarding psychiatric needs and issues, and assisting in the care of patients presenting with psychiatric crises or emergencies.
This position is considered open rank based on number of hours worked.
Key Responsibilities:
Provides medical assessment, diagnosis and treatment of patients. Performs preadmission screening of patients being referred by outside agencies; admission health assessments and writes admission orders.
Evaluates and treats patients scheduled for medical clinic. Performs annual health assessments. Performs urgent evaluations and treats unscheduled patients in medical clinical or on patient's unit, and arranges disposition.
Works with various members of the psychiatry treatment team to engage the patient in the treatment of their medical illnesses. Collaborates with other members of the medical staff and outside consulting physicians in the evaluation and treatment of patients. Refers patients to other medical providers and/or hospitals as necessary.
Communicates effectively with team members and other hospital staff.
Follows proper documentation policies including medical necessity for patient treatments. Remains up-to-date with documentation as described in policy.

Responds to medical emergencies and code zeros within the hospital while on duty.

Please note - a HCSP cannot provide any Skilled Care Services.
This position is based out of the Aurora/Denver Metro Area.
Shifts and locations will vary depending on the individual's needs with whom you are working. This is not a full-time position. This position will primarily provide in-home respite services to families and individuals that care for an individual with intellectual and developmental disabilities. The hours and schedule will vary depending on the need.
Requirements to be our Home and Community Support Provider
High school diploma or GED
Prefer one year experience in an area directly related to job assignment (i.e., developmental disabilities, elderly etc.)
Ability to pass and maintain certification in all agency required training
Ability to perform needed personal care
Ability to operate a wheelchair appropriately and assist with non-ambulatory clients
Ability to read, write and converse in English and perform simple math calculations for purposes of carrying out job assignments
Ability to perform basic food preparation as applicable
Ability to meet physical demands of the position
Ability to perform basic computer skills
Preferred
Personal vehicle in good operating condition for use during work as appropriate
Valid Colorado motor vehicle license
Proof of adequate motor vehicle insurance

Additional Responsibilities
Assist individuals in services in completing daily activities and other tasks (cleaning, toileting, meal preparation, meal pick-up and delivery, cooking, budgeting, hygiene etc.) as assigned
Implement and document services provided as outlined on the individuals in services plan of care
Carry out scheduled supported community connection activities as assigned (transports, provides supervision)
Provide care in accordance with established protocols, to include First Aid/CPR when needed
Assist in the development and implementation of individualized care plans
Document provider notes, incidents, and health and safety concerns as required

Primary Duties:
Maintaining a clean, safe, and healthy environment within client's residence. Duties may include light cleaning and straightening of the bathroom, straightening the sleeping and living areas, washing client's dishes or laundry, and other tasks to maintain cleanliness and safety for the client.
The performance of all personal care activities contained in a written Plan of Care which may include assisting the client with personal hygiene, ambulation and exercise, eating, dressing, shaving, physical transfer, and other duties as assigned.
Providing nutritional support as needed.
Reporting changes in the client's condition.
Other activities that are determined necessary and trained on for a specific client and are restricted to the following:
Assisting with the use of devices for aid to daily living such as a wheelchair, walker, or hoyer lift;
Assisting with prescribed range of motion exercises;
Teaching household routine and skills to well members of the family;
Measuring intake and output of fluids;
Measuring and recording temperature, pulse, respiration or blood pressure;
Keeping records of personal health care activities;
Observing appearance and gross behavioral changes in the client and reporting to the agency supervisor.
Assisting the client with the self-administration of medication, limited to the following tasks:
Taking the medication in its previously dispensed, properly labeled container, from where it is stored and bringing it to the client.
In the presence of the client, reading the label, opening the container, removing a prescribed amount of medication from the container, and closing the container.
Placing an oral dosage in the client's hand or placing the dosage in another container and helping the client by lifting the container to his or her mouth.
Returning the medication container to proper storage.
Keeping a record of when a client receives assistance with self-administration of medication.
The caregiver may also provide the following assistance with self-administered medication, as needed by the client:
Preparing necessary items such as juice, water, cups, or spoons to assist the client in the self-administration of medication;
Opening and closing the medication container or tearing the foil of prepackaged medications;
Assisting the client in the self-administration process. Examples of such assistance include the steadying of the arm, hand, or other parts of the client's body so as to allow the self-administration of medication;
Abides by the agency's infection control policies, including proper hand washing techniques consistent with CDC Guidelines.
Abides by all policies and procedures outlined in the agency's Policy and Procedure Manual, Caregiver Handbook, and Caregiver Training Manual.
Hours: Shift to START at 6:30am to 9:30a and option to to take 2 hour shift 10 minutes away. Shift Pay starts at $13 hour with option to take more shifts as well. Shifts available 24 hours a day, 7 days per week and based on availability. Assisting Hands does not guarantee hours.
