CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.
Job Summary:
Under the direction of the Operations Supervisor and Home Care Manager, and as a member of the PACE Interdisciplinary Team (IDT), the Home Health Aide utilizes their own personal, reliable transportation vehicle to visit PACE participants' private homes, apartments, etc. in order to assist with Activities of Daily Living, as prescribed in participants' plans of care and daily assignments. Because this is a home care position that will require driving to participants' living environments in order to provide care as scheduled, having reliable transportation is a requirement for this position.
Responsibilities:
Works independently in the private home, apartment, etc. of PACE participants
Utilizes their own personal, reliable transportation vehicle to reach the private homes of PACE participants
Assists participants in bathing, including complete, partial, or sponge bathing in tub or shower
Assists with laundry or provides laundry service as assigned
Ensures participants' homes/living environments are clean and tidy in order to promote health and safety
Communicates effectively with management, peers, and healthcare team members
Completes daily home care or day center assignment, providing support and assistance with activities of daily living (ADLs)
Qualifications:
High school diploma or equivalent
At least one year working with an older adult population required
At least one year experience working as a personal care attendant, home health aide, or other direct care professional required
Massachusetts drivers license and personal transportation required
Home Health Aide Certification in Massachusetts highly preferred

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
The Long-Term Support Services Coordinator (LTSSC) is a member of the Primary Care Team (PCT) and participates in Initial and Ongoing Assessments of the health and functional status of enrollees, including determining appropriateness for institutional long-term care services and developing community-based care plans and related service packages necessary to improve or maintain an enrollee's health and functioning.
The LTSSC arranges, coordinates, and authorizes the provision of appropriate long-term care and social support services for enrollees; with the agreement of the PCT and by requirements set forth by the plan. It may include but is not limited to ADL and IADL assistance, housing, home-delivered meals, and transportation.
The LTSSC monitors the appropriate provision and functional outcomes of community long-term care services, which the PCT has authorized.
The LTSSC tracks enrollee transfer from one setting to another (for example, hospital to home or nursing home to adult day health) and adjusts the service plan as deemed appropriate by the PCT and by requirements set forth by the plan.
The LTSSC participates in case conferences with the PCP, Nurse Care Managers, and representatives from other disciplines to identify the optimal care plan for plan members.
The LTSSC responds promptly to enrollee service requests.
The LTSSC collaborates with the PCT, the enrollee, the designated representative, and community-based service providers to determine appropriate discharge plans after admission to an institution.
The LTSSC prepares documentation for enrollee files and ensures appropriate and timely entries and updates of enrollee information available in the Centralized Enrollee Record (CER).
The LTSSC has a working knowledge of SAMS and plans CER to enter and retrieve data.
The LTSSC performs all services in accordance with state and federal law and each written plan's management protocols.
Report suspected Fraud, Waste and Abuse (FW&A) as required by CBES FW&A policy.
Protects Consumer's Personal Health Information (PHI) and reports any suspected security breaches.
The LTSSC attends and participates in agency and departmental meetings and training as required.

Bachelor's degree in social work or human services or at least two years working in a human service field with demonstrated experience working with the elderly/disabled population.

Griswold Home Care for Metrowest Boston is seeking compassionate, dedicated and experienced individuals to join our team as CNAs, HHAs, and Companions. This is a part-time, hourly healthcare/medical job. Responsibilities:
- Provide assistance with activities of daily living, such as bathing, dressing, grooming, and toileting
- Perform household tasks, including light housekeeping, laundry, and meal preparation
- Escort clients to appointments, errands, and social events
- Provide companionship and emotional support to clients
- Monitor clients' conditions and report any changes to the appropriate healthcare professionals
- Maintain accurate records of services provided and client progress
- Follow care plans and communicate any modifications to the care team
- Adhere to all safety and infection control protocols
- Meet all documentation and reporting requirements set forth by Griswold Home Care and regulatory agencies
Requirements:
- Active CNA, HHA or Companion certificate or license in the state of Massachusetts
- Minimum of 1 year of related experience
- Ability to pass a background check and drug screening
- Strong communication and interpersonal skills
- Compassionate and empathetic nature
- Ability to work independently and follow care plans
- Reliable transportation and valid driver's license
- Must be able to provide 2 professional references and 2 personal references
