These mobile services are provided in the home and community and aim to prevent the need for more restrictive and higher intensity services for children and youth. Psychosocial Rehabilitation (PSR) services are designed to restore, rehabilitate, and support a child's/youth's developmentally appropriate functioning as necessary for the integration of the child/youth as an active and productive member of their family and community with the goal of achieving minimal on-going professional intervention. Services assist with implementing interventions on a treatment plan to compensate for, or eliminate, functional deficits and interpersonal and/or behavioral health barriers associated with a child/youth's behavioral health needs. Activities are “hands on” and task oriented, intended to achieve the identified goals or objectives as set forth in the child/youth's individualized treatment plan. These services must include assisting the child/youth to develop and apply skills in natural settings. PSR is intended to foster and promote the development of needed skills identified in assessment or through the ongoing treatment of a licensed practitioner. Services are delivered in a trauma informed, culturally and linguistically competent manner. SPECIFIC DUTIES and RESPONSIBILITIES Engage the child and their family in a variety of strategies (including counseling and functional supports) to work toward identified goals. Engage in regular communication with the child's team, to ensure continuity of care. Document client progress and maintain a permanent record of client activity according to established methods and procedures. Practice with a person-centered, trauma informed philosophy in alignment with the values of Greater Mental Health of NY. Due to the nature of the program, must be able to work a flexible schedule. Other designated or related duties. REQUIRED KNOWLEDGE AND SKILLS: Knowledge of the signs of psychiatric crises and engagement skills/de-escalation techniques. Highly organized and self-motivated. Familiarity with and ability to use an Electronic Health Record system. QUALIFICATIONS: The candidate must be 21 years old and have a high school diploma, high school equivalency, or a State Education Commencement Credential with a minimum of two years' experience in children's mental health, addiction, foster care and/or a related human services field; OR a Bachelor's degree in social work, psychology, or in related human services; OR a Master's degree in social work, psychology, or in related human services. Basic Computer knowledge is required. Must be reliable and able to work independently and understand the importance of maintaining confidentiality. The PSR Worker must have a valid Driver's License and a driving record that is satisfactory to our insurance carrier. The PSR worker will be using agency vehicles and is responsible for adhering to Agency Driver Policy. The PSR must maintain current New York State Defensive Driving certification. The PSR Worker must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search. The PSR Worker must be cleared by the NYS Office of Children and Family Services for instances of child abuse and/or neglect. The PSR Worker must be legally eligible to work in the United States. MercyFirst's Qualified Residential Treatment Program (Group Home) provides 24-hour residential care and supportive services for youth with a history of trauma and the presence of complex needs. Three agency-operated group homes provide youth with structure and supervision within community-level settings designed to provide evidence-based treatment to promote emotional and physical safety and stability, practice positive coping skills, and build healthy relationships, as well as case management services to support discharge planning through family reunification or independent housing, and youth development services to foster growth and competency in daily life skills . POSITION SUMMARY: The Aftercare worker for the QRTP program will be responsible for assessment, engagement and case management services for clients and families towards successful transition out of QRTP placement. This qualified candidate will be a part of a multi-disciplinary team and work alongside the QRTP caseplanner and will refer guardian for supportive services, including medical, clinical and concrete services. Upon the youth' transition out of the QRTP program, the Aftercare Caseworker will provide the youth and their family with continued casework support up until the youth's placement order expires, to ensure successful reintegration into the community and in the home. Case planner will present the program's efforts in legal proceedings, ACS/OCFS meetings and other interagency conferences. The qualified candidate will complete the required progress notes, assessments, Psychosocial reports, FASPs, court reports, etc within timely mandates. RESPONSIBILITIES: Conduct regular casework sessions with each client in accordance with Federal, State, and Local mandates. While youth is in placement: minimal monthly meeting with youth and family. Meet with youth 2x weekly/legal guardian weekly for the first 6 weeks; once weekly thereafter. Home assessments within the first week of youth's placement, monthly thereafter. Collateral contact weekly with youth's school/therapist/other resources. These mobile services are provided in the home and community and aim to prevent the need for more restrictive and higher intensity services for children and youth. Psychosocial Rehabilitation (PSR) services are designed to restore, rehabilitate, and support a child's/youth's developmentally appropriate functioning as necessary for the integration of the child/youth as an active and productive member of their family and community with the goal of achieving minimal on-going professional intervention. Services assist with implementing interventions on a treatment plan to compensate for, or eliminate, functional deficits and interpersonal and/or behavioral health barriers associated with a child/youth's behavioral health needs. Activities are “hands on” and task oriented, intended to achieve the identified goals or objectives as set forth in the child/youth's individualized treatment plan. These services must include assisting the child/youth to develop and apply skills in natural settings. PSR is intended to foster and promote the development of needed skills identified in assessment or through the ongoing treatment of a licensed practitioner. Services are delivered in a trauma informed, culturally and linguistically competent manner. SPECIFIC DUTIES and RESPONSIBILITIES Engage the child and their family in a variety of strategies (including counseling and functional supports) to work toward identified goals. Engage in regular communication with the child's team, to ensure continuity of care. Document client progress and maintain a permanent record of client activity according to established methods and procedures. Practice with a person-centered, trauma informed philosophy in alignment with the values of Greater Mental Health of NY. Due to the nature of the program, must be able to work a flexible schedule. Other designated or related duties. REQUIRED KNOWLEDGE AND SKILLS: Knowledge of the signs of psychiatric crises and engagement skills/de-escalation techniques. Highly organized and self-motivated. Familiarity with and ability to use an Electronic Health Record system. QUALIFICATIONS: The candidate must be 21 years old and have a high school diploma, high school equivalency, or a State Education Commencement Credential with a minimum of two years' experience in children's mental health, addiction, foster care and/or a related human services field; OR a Bachelor's degree in social work, psychology, or in related human services; OR a Master's degree in social work, psychology, or in related human services. Basic Computer knowledge is required. Must be reliable and able to work independently and understand the importance of maintaining confidentiality. The PSR Worker must have a valid Driver's License and a driving record that is satisfactory to our insurance carrier. The PSR worker will be using agency vehicles and is responsible for adhering to Agency Driver Policy. The PSR must maintain current New York State Defensive Driving certification. The PSR Worker must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search. The PSR Worker must be cleared by the NYS Office of Children and Family Services for instances of child abuse and/or neglect. The PSR Worker must be legally eligible to work in the United States. Department Overview The Residential Crisis Support Program Respite Center (CRC) is a Community Access Health Division program. The CRC is an alternative to hospitalization program offering short-term respite care in a friendly and supportive home-like environment. Guests are provided with the opportunity to learn and use recovery and relapse prevention skills to ameliorate their current psychiatric crisis and to provide more effective tools to cope with future crises and avoid hospital-based care. Recovery-oriented services are provided 24-hour and day and include peer support, self-advocacy education and self-help training. The Residential Crisis Support Program offers people experiencing a mental health or emotional crisis an up to 28 days residential alternative while supporting individuals in continuing their established daily activities such as visiting with friends, attending a doctor's appointment or going to work. Position Overview The Per Diem Respite Worker is responsible for all tasks related to the Respite daily operation during shift hours including providing services and support to CRC guests. Some responsibilities of the Respite Worker are to develop partnerships with guests experiencing crisis utilizing trauma-informed, recovery-oriented intentional peer support and promotes the provision of a comfortable, secure, and trusting environment where guests are listened to and respected by staff.
