    Part-Time 0800-1630 Assumes accountability and responsibility for managing the hospitalization of a caseload of patients from pre-admission through one week post-discharge. Facilitates coordination of care by all members of the health care team to facilitate achievement of appropriate outcomes. Responsibilities: 1. Evaluates patient needs, provides case management. 2. In collaboration with the M.D. /CNS, provides direct patient care to patients, including, but not limited to, medication management follow-up and decision-making about follow-up care, crisis stabilization, medication changes, and wraparound services. 3. Assists in the planning and implementation of medical care and coordination of services with other patient care units, both within the hospital and through outside institutions and links families with community resources. 4. Care is established from the time of admission to the service through termination by utilizing the nursing process in such a manner as to maximize outcomes. 5. Additional duties include the development and implementation of materials necessary to facilitate patient care with varied care providers, patient and family education, staff and care provider education, development of patient and family educational materials. 6. Participation in research, basic supportive counseling, procurement of community resources for families, and assessment of changing patient needs throughout the course of care with appropriate referrals for consultation as needed. Demonstrate leadership and interpersonal relationship skills which enhance communication and promote conflict resolution. Job Summary Molina Healthcare Services (HCS) works with members, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health and long term care, for members with high need potential. HCS staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service. KNOWLEDGE/SKILLS/ABILITIES Completes clinical assessments of members per regulated timelines and determines who may qualify for case management based on clinical judgment, changes in member's health or psychosocial wellness, and triggers from the assessment. Develops and implements a case management plan in collaboration with the member, caregiver, physician and/or other appropriate healthcare professionals and member's support network to address the member needs and goals. Conducts telephonic, face-to-face or home visits as required. Performs ongoing monitoring of the care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly. Maintains ongoing member case load for regular outreach and management. Promotes integration of services for members including behavioral health care and long term services and supports to enhance the continuity of care for Molina members. May implement specific Molina wellness programs i.e. asthma and depression disease management. Facilitates interdisciplinary care team meetings and informal ICT collaboration. Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts. Assesses for barriers to care, provides care coordination and assistance to member to address concerns. Collaborates with RN case managers/supervisors as needed or required Case managers in Behavioral Health and Social Science fields may provide consultation, resources and recommendations to peers as needed Local travel of up to 40% may be required, depending on the complexity level of the assigned members, particular state-specific regulations, or whether the Case Manager position is located within Molinas Central Programs unit. This position is critical to the delivery of quality psychosocial care and management of patients receiving cancer care at our regional sites. A caregiver in this position works the day shift from 8:00AM to 5:00PM. A caregiver who excels in this role will: Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems. Perform psychosocial assessments and provide interventions to assist patients and families. This may include counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings. Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans. Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers. Maintain a network of resources for consultation and connect patients with community resources. Supervise social work interns and assist with training new caregivers. Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes. Minimum qualifications for the ideal future caregiver include: Master's degree in social work from a CSWE accredited program. State Licensed Social Worker (LSW). Two years of experience in a hospital or healthcare setting. Licensed Independent Social Worker (LISW) within five years of hire.
