5. Additional duties include the development and implementation of materials necessary to facilitate patient care with varied care providers, patient and family education, staff and care provider education, development of patient and family educational materials.
6. Participation in research, basic supportive counseling, procurement of community resources for families, and assessment of changing patient needs throughout the course of care with appropriate referrals for consultation as needed.

During their stay, patients develop self-management and recovery skills that help them rebuild their lives. We are looking for a Residential Counselor to provide direct service while the patients are staying at RAMAR. Counselors provide individual and group counseling by evaluating assessments for identified patients and their family members and based on findings, facilitating a timely and appropriate treatment plan, and coordinating said plan with available agency services and the community. The Opportunity: We are seeking to hire an ACT Counselor to join our Assertive Community Treatment (ACT) Team. This individual provides group and individual clinical interventions for persons with severe and persistent mental illnesses, substance use disorders and or co-occurring medical conditions in congruence with team fidelity standards for ACT Unit clients.
What You'll Do:
Provide individualized, stage wise, and harm reducing counseling services to meet the needs of the clients with severe and persistent mental illnesses and substance use disorders that are served by ACT treatment team.
Develop and facilitate therapeutic groups to meet the needs of the clients served by ACT treatment team, with primary emphasis on programming for older adults with severe and persistent mental illness and substance use disorders. Development and facilitation of support/educational groups for client's families included.
Provide counseling services, office and community based, within credentials and competency, to participants under the supervision of Assertive Community Treatment Manager.
Develop the individual's service plan in accordance with established agency procedures. Performs crisis resolution using the least restrictive intervention.
Coordinate services with other providers and consults with other treatment team members as appropriate.
Provides cross training to ACT treatment teams on basic counseling interventions, skills, and communication and other topics as deemed appropriate.
Maintain relevant documentation in accordance with agency documentation standards.
This position is full-time (40 hours a week) working normal business hours.

Master's degree in a Social Sciences field required.

Maintain a basic understanding of the differences between indigent case and estate cases.
Completes the appropriate application for the Probate Court within 10 days of the assessment.
Communicates and provides up to date accurate information to the Attorney assigned to the case within the required time frame.
The caseworker will assume on call duties as assigned during non-business hours. Periodically they may need to provide transportation for a ward. On occasion, the caseworker may need to respond to a crisis matter on a weekend or during the evening hours. Other duties as assigned by supervisor.

Gather third party information during the assessment to collaborate need to Guardianship.
Contact family and significant other to discuss and gather information for the Guardianship application process.
Informs the court of significant changes in the case and files required reports with the court in a timely fashion.
Maintain a basic understanding of entitlements and benefits programs and how to apply for needed benefits.
Understanding Medicare and Medicaid rules as they pertain to nursing home placements, spend downs, and medical services provided under the ward's applicable insurance carrier.
Maintain a basic understanding of the differences between indigent case and estate cases.
Completes the appropriate application for the Probate Court within 10 days of the assessment.
Communicates and provides up to date accurate information to the Attorney assigned to the case within the required time frame.
The caseworker will assume on call duties as assigned during non-business hours. Periodically they may need to provide transportation for a ward. On occasion, the caseworker may need to respond to a crisis matter on a weekend or during the evening hours. Other duties as assigned by supervisor.
Qualifications:
Degree in Social Work or bachelor's degree in finance or legal professional preferred
LSW or LPC licensure is beneficial
Prior experience working with probate court, estates, financial accounting and or retail banking operations
Strong working knowledge of issues which impact the elderly and the mentally ill.

GENERAL STATEMENT OF DUTIES: The Social Service Assistant will be responsible to the manager of the locality within which they are employed and work in conjunction with the Senior Social Service staff to provide a safe and stimulating environment for residents. They will be expected to work within the policies and procedures of Family & Community Services and work toward the maintenance of excellent service standards and professional practices.
ESSENTIAL RESPONSIBILITIES:
1. To assist in the efficient running of the facility and its therapeutic programming and other general duties associated.
