The HRC NP will serve as an integral member of a multidisciplinary team that provides substance use treatment in a large, federally qualified health center in downtown Brockton. They are champions of collaborative, patient centered care that is inclusive of harm reduction strategies and medications for addiction treatment (MAT). Principle Duties and Responsibilities:
Provides direct patient care for substance use and co-occurring mental health disorders, both in-person and via telemedicine, that is inclusive of a harm reduction framework
Works collaboratively as a part of a multidisciplinary team including nurse care managers, behavioral health clinicians, recovery coaches, and other support staff
