Daily Responsibilities for Pharmacist:
Fills prescription items, as permitted by the State Board of Pharmacy, packaging medications for dispensing and for delivery, meeting the accuracy and productivity standards of the pharmacy
Labels products appropriately, including cautionary or auxiliary labels
Participates in the periodic inventory of the pharmacy
May check in drugs and supplies, stock shelves and remove out-of-date items from the inventory
Prepares medications including emulsions, liquids, powders and ointments, which may include non-sterile compounding
Communicates with internal and external customers in a pleasant and professional manner
May process returned medications from the facility for credit or destruction
Assists in the training of new employees
Also performs at least one of the following:
Works in the Controlled Drugs (Narcotics) Room;
Works with automated machine to prepack drugs;
Manages inventory functions such as monitoring pharmaceutical levels and identify expired medications, managing to the pharmacy standard for Days on Hand;
Inputs patient data and medication orders into a computer system to produce labels (data entry);
Mixes/packages IV medications, sanitizes IV room, equipment and hood
Services facility-based medication dispensing machines (e.g., RxNow)
Performs other tasks as assigned

Required Qualifications for Pharmacist :
Required: High School diploma or equivalent.
Experience in a pharmacy setting and with medical terminology.
Licenses/Certifications: As required by state regulations. Not required in state of PA.
Long Term Care pharmacy experience preferred.

2+ years' experience as a Pharmacist in a specialty pharmacy setting

SUMMARY: The Pharmacist will provide accurate medication packaging and distribution services to Adult/Youth in long term care facilities. Communicating with a multidisciplinary health care team, the Pharmacist will ensure that each Adult/Youth's prescription therapy is safe and appropriate. Medication Therapy Management services performed by the Pharmacist will enhance patient safety and healthy lifestyles.
DUTIES AND RESPONSIBILITIES:
Verify prescriptions entered in pharmacy system including correct patient, drug and dose.
Check for drug-drug/drug-disease interactions to prevent adverse drug reactions.
Communicate with prescribers to clarify unclear or potentially harmful orders.
Council patients and/or caregivers on any adverse drug reactions, interactions with other medications or food and alcohol.
Advise patients and/or caregivers about general health topics - diet, exercise, stress management, etc.
Interact and engage with insurance companies and prescribers to ensure payment and resolve coverage denials.
Complete a medication reconciliation when there are transitions in care to resolve medication discrepancies and ensure patients are getting the correct medication regimen.
Perform Medication Therapy Management services including comprehensive medication reviews, interventions and education. Bill for these services.
Manage staff overseeing and mentoring pharmacy technicians, student interns and residents.
Maintain accurate inventory of medications.
Educate health provider colleagues about proper medication therapies for patients.
Oversee the operation of automated dispensing system and verification system.
Supervise and/or complete the final check of finished product for accuracy
Assure medications and medication cycles are delivered timely
Other duties as assigned.
Provide on-call services afterhours on a rotational schedule.
Comply with all applicable legal rules, regulations and procedures.
SUPERVISORY RESPONSIBILITIES:
The Pharmacist is responsible for the active and timely supervision of all direct and/or indirect reports within his or her department including pharmacy technicians.
QUALIFICATIONS:
Be a Registered Pharmacist in good standing in the State of Wisconsin;
Possess a Bachelor's degree in Pharmacy. A Doctor of Pharmacy is preferred;
Meet all licensing requirements set forth by the respective state of operation;
Possess a valid, regular (non-probationary) state driver's license if the individual will be driving either their personal vehicle for on-the-job purposes or any agency vehicles;
Possess personal vehicle insurance that meets the minimum expectations/coverage if the employee will be driving their personal vehicle for on-the-job purposes as detailed in the BBH Transportation Policy;
Meet the background check requirements for each state's regulations;
Successfully pass an initial medical examination and pass a drug screening, which provides evidence that they are free from communicable diseases, including active tuberculosis or conditions which prohibit them from reasonably performing the assigned duties, except as provided for by the ADA. Staff will be reexamined on a regular basis.
Have a minimum of two (2) years of supervisory experience in a Long Term Care pharmacy environment (preferred);
Have WPQC (Wisconsin Pharmacy Quality Collaborative) certification, or obtain certification within 180 days of assuming Pharmacist role.

Any 8hr shift between 7 am to 7:30 pm, it will rotate every 2 weeks Note that hours could be anytime between 6am and 10pm (usually 8 hour shift) Mon-Friday - 8 hour shift.
