Then update Coumadin Management Flowsheet as required and renew warfarin prescription to ensure patient has sufficient supply. Works collaboratively with referring primary care providers for the management of smoking cessation patients. Pharmacist will call patients are needed when due for follow-up and ensure patient has sufficient supply of smoking cessation aid. Conduct prospective and retrospective chart reviews on selected patients and discuss findings with providers and/or Chief Medical Officer. Maintains confidentiality of all pertinent information to assure that patient and staff rights are protected. Communicates any changes to the facility's Medical Assistant for review Reviews all new admissions daily and ensures that medications are billed to the appropriate insurance carriers. Communicates all information the billing department Is an integral member of the facility's committee's; Pharmacy and Therapeutics Committee, Antibiotic Stewardship committee, and Infection Control committee, and provides expertise as requested Supervises all medication prior authorizations and Interchange request, to ensure that they are completed effectively and in a timely manner Supervises all medication returns to ensure quick return and facility credit Provides for the educational needs of all health care professionals through periodic in-services at the request of the facility Ensures maintenance of an adequate medication supply, of all emergency boxes, house stock inventory, crash carts, Automatic Dispensing Cabinet (ADS) inventory, and control medications, specifically (methadone) Ensures the integrity of the medication supply Establishes specifications for the procurement of medications, chemicals, and biologicals Ensures strict control and accountability for control medications (Methadone) distributed to floor stock Assures adequate control and documentation of controlled substances Ensures applicable continuing education records and licensure are maintained in department files Supervises all pharmacy personnel, disciplines and terminates pharmacy staff. Develops job descriptions and performance standards. Evaluates and counsels' staff on their performance Prepares work schedules and monitors workload statistics.
