Supports and serves as consultant to all clinical and Medical Staff. Assists the Director in coordinating and implementing the activities of the Pharmacy. Interprets medication orders, compounds and dispenses medication and other pharmaceutical preparations in accordance with the legal, ethical and professional standards of pharmacy practice. Supervises the technical support staff. Assists with clinical, quality management and education programs. Provides support for all departments with respect to the care of all patients aged 18 through the geriatric population ages 65 and older. Clinical duties include renal dosing, drug regimen review, antibiotic & anticoagulant monitoring. Must be willing to provide on-call pharmacist services when needed Pharmacist, Pharm D, hospital pharmacy, LTAC, Transitional Care, Acute CareQualifications Graduate of an accredited School of Pharmacy with Pharmacist degree. Current state license in good standing 3 years of experience preferred in HOSPITAL pharmacy. Pharm D or residency preferred. Working knowledge of OSHA, JCAHO, HCFA and state guidelines. In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for providing pharmaceutical care services. Responsibilities Ensure that prescriptions are dispensed safely and accurately as prescribed. Conduct patient drug regimen review. Provide patient prescription counseling services as required. Prepare and maintain required records as per requirements. Communicate with patients, providers and other clinical staff as needed. Perform other duties as assigned.
