The Pharmacist serves patients by safely preparing medications; provides pharmacology and research protocol information to multidisciplinary health care teams and researchers; adheres to NYSBP, TJC, NYDOH, DEA, FDA, NIH, and all other applicable regulatory agency requirements. To be in compliance, the following measures must be taken: Monitor nursing unit inspections; maintain records of all controlled substances; remove outdated and damaged drugs from the pharmacy inventory; supervise the work results of support personnel; maintain current pharmacist registration; study existing and new legislation; anticipate legislation; maintain investigational and commercial drug inventories, dispense research drug products according to protocol, advise management on needed actions, maintain required aseptic procedures, maintain the sterile product preparation area, maintain refrigerator/freezer temperature records, and other tasks as indicated by the Pharmacy Director or other supervisor; Contributes to team effort by accomplishing related results as needed; Perform additional duties as needed/assigned. Qualifications Required Qualifications: At least 2 years working in a hospital pharmacy, preferably with significant activities involving both investigational drug products and sterile product preparation; NYS Pharmacist license in good standing; Proficient with hospital-based electronic health records, drug order entry systems, and controlled drug inventory systems; Experience dispensing controlled drugs in a hospital setting and automated drug dispensing machines; Ability to read and understand clinical trial protocols and dispense drug products (both sterile and non-sterile products) according to protocol instructions. Must be able to do this without supervision; Ability to maintain pharmacy records in an ongoing manner and write drug dispensing procedures without supervision; Must be able to commit to a full-time onsite, not hybrid, work schedule. The job requires the pharmacist to participate in the pharmacy on-call schedule, which provides pharmacist availability 24/7 for the Hospital. When called, the pharmacist is required to be able to both remotely connect with the Hospital EHR system and, if necessary, travel to the hospital to troubleshoot and solve any problems; Must be able to have flexibility with schedule to accommodate planned medication administration times; Ability to work as part of a research team in an academic environment, interest in publishing and writing articles involving pharmacy and science, write articles and collaborate on projects for publication; Experience and interest in mentoring pharmacy interns and extern students. This is a per diem position. OHI is switching to ten-hour shifts in January, 2025. Candidate must be able to commit to this schedule in 2025. . Reporting to the Pharmacy Coordinator, the Pharmacist will provide safe and effective pharmacy services in addition to dispensing medications. These services include the provision of pharmaceutical care, medication dispensing, patient counseling, drug information services and patient and staff education. Responsibilities Provide patient education on medication and reason for use, frequency and dosages, side effects, and desired therapeutic effects. Enhance educational experience using drug information flyers or pamphlets appropriate for patient level of health literacy. Fills in-house generated prescriptions in accordance with all federal regulations and all state regulations as per the NJ Board of Pharmacy. Fills outside provider prescriptions in accordance to OHI Outside Prescription Policy in addition to all federal and state regulations as per the NJ Board of Pharmacy. Serves as a resource of information on medications, pharmacokinetics and interactions, to providers. Assist providers in the management of patients with polypharmacy including conducting medication reconciliation, writing medication lists, completing the patient medication card, and providing other means for patients to understand how to take their medications. Works collaboratively with referring primary care providers for the management of smoking cessation patients. Pharmacist will call patients are needed when due for follow-up and ensure patient has sufficient supply of smoking cessation aid. Conduct prospective and retrospective chart reviews on selected patients and discuss findings with providers and/or Chief Medical Officer. The pharmacist on duty will always attempt to purchase the least expensive generic medication available (depending on availability). Maintains and updates the pharmacy formulary when needed. Works with Pharmacy Coordinator on any issues pertaining to the practice of pharmacy, pricing of medications and ordering of supplies or any issues that require attention. Utilizes the Liberty help desk to address any pharmacy software issues that cannot be handled by their immediate supervisor and shares that information with all pharmacy staff. Utilizes PBM help desks to address any 3rd party prescription rejection claims. Designates appropriate tasks and projects to pharmacy technicians and holds techs accountable for completion. Assures pharmacy refrigerator/freezer logs are completed daily and filed accordingly. Compares and rectifies daily McKesson invoices with the automatically generated RXQ (Liberty) order and assures the dollar value is equal.
