Primary Role:
Preparing and dispensing appropriate pharmaceuticals to patients. The job entails a high level of responsibility and knowledge
Required Education & Skills:
• Active PharmD or RPH licensure to practice in PA
• Ability to provide professional advice on medications dispensed and sold over the counter
• Ability to provide effective medication use and become the people's trusted ally against health issues
Experience:
• Experience with the utilization of Patient Management System in the Pharmacy
• 2 years related pharmacy experience in Pharmacy Management
• 340B experience Preferred
• Spanish Bilingual preferred
Essential Job Functions:
• Review and execute provider's prescriptions checking their appropriateness and legality
• Organize the pharmacy in an efficient manner to make the identification of products easier and faster
• Maintain full control over medication inventory, stocking and labeling medicine and other products and monitor their condition to prevent expiring or deterioration
• Listen carefully to customers to interpret their needs and issues and offer information and advice.
• Prepare medicine when appropriate using correct dosages and material for each individual patient
• Maintain patient profiles and history of all activities regarding medications dispensed
• Keep abreast and stay current with HIPAA and Frauds Waste and Abuse credentialing
• Comply with all applicable legal rules, regulations, and procedures
• Responsible for ensuring that the Pharmacy and its employees are compliant with Federal, State and Local laws regulated
• Ensures that the Pharmacy and its employees are aware and compliant with local facilities policies
• Responsible for directing the daily work of Pharmacy Technicians, this includes providing input into performance evaluations and advising Senior Management of any behavioral and/or performance issues.

This is a per diem position. OHI is switching to ten-hour shifts in January, 2025. Candidate must be able to commit to this schedule in 2025. .
Reporting to the Pharmacy Coordinator, the Pharmacist will provide safe and effective pharmacy services in addition to dispensing medications. These services include the provision of pharmaceutical care, medication dispensing, patient counseling, drug information services and patient and staff education.
Responsibilities
Provide patient education on medication and reason for use, frequency and dosages, side effects, and desired therapeutic effects. Enhance educational experience using drug information flyers or pamphlets appropriate for patient level of health literacy.
Fills in-house generated prescriptions in accordance with all federal regulations and all state regulations as per the NJ Board of Pharmacy.
Fills outside provider prescriptions in accordance to OHI Outside Prescription Policy in addition to all federal and state regulations as per the NJ Board of Pharmacy.
Serves as a resource of information on medications, pharmacokinetics and interactions, to providers.
Assist providers in the management of patients with polypharmacy including conducting medication reconciliation, writing medication lists, completing the patient medication card, and providing other means for patients to understand how to take their medications.
Works in the CEC periodically to assist providers with requests from pharmacies regarding therapeutic interchanges, questions regarding prescriptions and continuity of care to ensure patients get prescriptions in a timely manner.
Work collaboratively with the Coumadin Management Team to evaluate, assess and assist patients with their INR once notified of results by nurse. Then update Coumadin Management Flowsheet as required and renew warfarin prescription to ensure patient has sufficient supply.

ESSENTIAL RESPONSIBILITIES/DUTIES:
Patient Relationship Development and Client Sales - You will focus on sales/customer service with the goal of expanding your patient account base by enrolling and retaining specialty pharmacy services offered through Clearway Health.
Sales and Enrollment
· You accomplish set goals for enrolling patients into the Clearway Health Specialty Pharmacy program. Your success is measured consistently and evaluated through various sales and reporting platforms, focusing on customer relationships and the revenue performance of each individual specialty service line.
· You are responsible for performing enrollment functions, including following up on provided leads for new business and generating leads through your provider relationships or direct customer service engagements.
Patient Retention
· You build relationships with patients by offering them friendly, courteous, and efficient service, all while encouraging them to use Clearway Health to access its top-notch patient care services.
· You work closely with patients, delivering a full continuum of medication adherence support through various tools and applications.
· You provide outbound therapy/medication adherence check-ups by processing refills to eliminate any gaps.
Personnel Responsibilities - You are responsible for building and maintaining relationships with providers, care teams, and ancillary support to ensure the continuity of sales and enrollment functions.
Internal Relationships
· You develop and nurture trusting relationships to deliver the highest level of care for complex patients.
· You maintain and support pharmacy interdepartmental relationships and management, ensuring a smooth transition of patient care from clinic to pharmacy.
External Relationships
· You take the lead in creating and developing strategic partnerships with various healthcare professionals, providing them with the necessary information and support to ensure successful patient care transitions.
Operational Responsibilities - You are technically savvy, a natural problem-solver, and an effective communicator at all levels, both internally and externally.
· You skillfully navigate the patient Electronic Medical Record (EMR) to answer questions and provide documentation as needed for specialty pharmacy prior authorizations.
· You maintain effective communication with various healthcare professionals, such as doctors, nurses, clinical pharmacists, and practice managers, both in person, over the phone, and through written transmissions via EMR. Your interactions are consistently timely and professional, supporting new and ongoing initiatives.
· You excel at resolving high-level patient care issues, knowing when to involve managers or clinicians when needed. Your resourcefulness shines through when facing challenges, offering practical solutions to complex problems.

EDUCATION:
· High School diploma or G.E.D. required, higher level education preferred.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED:
· Active and unrestricted Pharmacy Technician license with the Board of Pharmacy in the respective state of role
· Active and unrestricted National Certification verified by PTCB or ExCPT.
EXPERIENCE:
· 2+ years of direct pharmacy experience required.
KNOWLEDGE AND SKILLS:
· Excellent verbal and written English communication skills required; as well as ability to communicate professionally over the phone.
