Duties and Responsibilities Patient Assessment: Conducting thorough assessments of patients to determine their suitability for home infusion therapy. This may involve reviewing medical records, evaluating lab results, and assessing the patient's overall health status In collaboration with the nursing department, ensure employing a home health nurse prior to the start of care if appropriate Collaborate with healthcare providers, including physicians, nurses, pharmacists, and discharge planners, to coordinate the transition of patients from hospital or clinic-based care to home infusion therapy Collaborate with the billing department, verify patient's insurance coverage and ensure that the prescribed medication is covered, obtain necessary approvals or explore alternative options if coverage is not available Review prescription orders for accuracy, completeness, and appropriateness to ensure that the prescribed medication is safe and effective for the patient Communicate with healthcare professionals to clarify prescription details or obtain additional information. RESPONSIBILITIES * Develop and document therapeutic plans, evaluate medication therapy * Order, review, and analyze laboratory test and other diagnostic studies necessary to monitor and support the patients medication therapy * Prescribe medications, devices, and supplies to include: initiation, continuation, discontinuation, monitoring, and altering therapy based on established formulary protocols and in accordance with P&T approved scope of practices * Order consults, as appropriate, to maximize drug therapy outcomes and disease state management * Monitor the patients response to drug therapy through all available sources, such as, patient a pharmacy records, and laboratory data * Educate and counsel the patient regarding his/her drug therapy and/or protocol QUALIFICATIONS * Bachelors Degree or Higher in Pharmacy * Pharmacy License * 2 year of experience as a Clinical Pharmacist performing direct patient care clinical activities in an inpatient, outpatient, or ambulatory care pharmacy * setting * Basic Life Support (BLS) * US Citizen * NACI Background check

Under the supervision of a licensed pharmacist and following departmental procedures, the pharmacy intern assists the pharmacists in all aspects of patient care. Duties include the processing physician medication orders for infant, pediatric, adult, and geriatric patients, ensuring medications are promptly, accurately, and safely delivered, charged for, and credited. Prepares various drugs and IV solutions for delivery to patients, and properly labels pharmaceutical orders. Maintains records related to drug use and charges. The pharmacy internship objectives of the California State Board of Pharmacy are met during the course of the job duties and responsibilities. Is proficient in required competencies.
