Responsibilities:
As a Pharmacist, you will...
Evaluate and monitor drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
Adhere to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.

Qualifications:

Pharmacist PRN (Mon-Fri Day Shift) - Ambulatory Care (Community Care Clinics) Our department's pharmacists are unique and significantly different from those in a typical hospital or retail pharmacy setting. As part of our embedded pharmacy care team at the community care clinics, the pharmacist works closely with a team of pharmacy technicians, pharmacist residents and pharmacy students to provide direct patient care. The community care clinics present a multidisciplinary team that provides outpatient care to patients who are underinsured, uninsured, and/or indigent. In addition to the pharmacy care team these clinics also presents behavior health, chronic disease, and navigation care teams as well as a part of a multidisciplinary team approach. Pharmacists work under a collaborative practice agreement to provide chronic medication management for patients with diabetes, hypertension, and other chronic disease states. Additionally, pharmacists help patients with adherence, education, medication affordability, reconciliation, and smoking cessation consultations. As part of our efforts to assist with the affordability of medications, pharmacists may also provide formulary and pricing information to patients and providers. Prescription assistance programs (PAPs) are another important resource we use extensively because the majority of our patients are either underinsured or uninsured. We use a database to track PAPs, and therefore some familiarity with PAP programs is preferred.

ProSidian seeks a Registered Pharmacist to support engagement with The Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP). This position serves as a key member of the facility leadership team providing services that include, but are not limited to, pharmaceutical verification of prescription orders of legend, over the counter (OTC), medical-surgical (Med/Surg), and controlled substances to patients requiring care. In addition, the candidate shall provide patient consultation, quality improvement, and supply control duties.

ProSidian seeks a Registered Pharmacist to support engagement with The Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP). This position serves as a key member of the facility leadership team providing services that include, but are not limited to, pharmaceutical verification of prescription orders of legend, over the counter (OTC), medical-surgical (Med/Surg), and controlled substances to patients requiring care. Education:

Pre-verifies medications loaded into highly specialized mechanical apparatus to dispense medications into prescription bottles.
Completes verification and fill of controlled substance orders, including Schedules III, IV, and V.
Performs Quality Assurance functions and supervision of Pharmacy Technicians.
Follows CMOP policies regarding documentation and reports to meet VA and TJC policy.
Notes any unusual doses, wrong drug, wrong strength, or drug mix up, and reports the information in accordance with local CMOP policies.
Provides full checking, labeling, and quality assurance within the guidance (written VHA handbooks, written CMOP policies, and training that will be provided by the CMOP) of the Department of Veterans Affairs regarding pharmaceutical medications.
Compounds and dispenses medications within corporate and regulatory guidelines for controlled substances.
Qualifications
Licensed in a State, Territory, or Commonwealth of the United States or the District of Columbia.
Must be a graduate of an American Council on Pharmaceutical Education (ACPE) accredited school or university. Pharmacists who are licensed but have been subjected to disciplinary action of any type by their licensing authority within the preceding five years, must be referred to CMOP management before proceeding with the hiring action.
Must be licensed for controlled substances, with qualifications to practice in any states.
Must demonstrate effective interpersonal communication skills with co-workers and staff.
Maintains basic typing skills, PC computer skills, and working knowledge of MS Office.
Must read, understand, speak, and write English fluently under the authority of U.S.C. 7402(d) and 7407(d).
Able to pass VetPro certification, which is a credentialing system that is required of all VHA licensed, registered, and certified health care providers.
Able to sit/stand for long periods of time (8-10 hours), and lift totes weighing up to 30 pounds repetitively.

Lift up to 20 lbs
Carry up to 40 lbs a reasonable distance
