Under the general supervision of the Manager, Ambulatory Pharmacy Services, and within established organizational and departmental policies and procedures, the Ambulatory Pharmacist II is responsible for providing pharmaceutical care in an institutional outpatient practice setting. Primary, essential job functions involve ensuring rational drug therapy through a focus on prescription filling/distribution, medication therapy monitoring, staff and student training, and project coordination. Must be willing/able to support the clinical pharmacy and other organizational activities as required. A commitment to patient safety, customer satisfaction, efficient/effective use of resources, teamwork, innovation, and performance improvement is required.
Job Purpose: • Checks for safety, accuracy, appropriateness and completeness with respect to drug, dose, route, frequency with consideration given to patient-specific data including laboratory values, age, gender, height, weight and BSA. • Prepares controlled substances to be dispensed, completing the appropriate documentation while maintaining an accurate perpetual inventory. • Performs weekly inventory and reconciliation of controlled substances. Notifies Manager, Ambulatory Pharmacy Services or Senior Director of Pharmacy of any unexplained discrepancies. • Performs daily oversight of all Pharmacy Technician activities; checks/reviews all Pharmacy Technician work according to regulatory requirements. • Dispenses medication to ambulatory patients with appropriate medication counseling. • Educates/trains patients to administer and monitor therapy as required. • Effectively interacts with prescribers concerning drug order clarification or drug therapy modification • Ability to perform without direct professional supervision and to oversee the work of Technicians and students.
Qualifications/Requirements:
Education/Training: Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree is required.
Licensure: Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA. In lieu of an active MA license, candidates must attain MA licensure prior to working 1000 hours at the Cambridge Health Alliance (CHA). Failure to attain MA licensure within this time period will result in the discontinuation of employment at CHA.
Work Experience: Minimum of five (5) years experience as a pharmacist is required.

A Day in the Life of this Role
The Pharmacist-in-Charge ("PIC") equates to the person who, being licensed as a pharmacist in the state that the pharmacy operation is physically located in, signs the application for the pharmacy permit and to whom the permit is issued to. Established in 1980, Greater Lawrence Family Health Center (GLFHC), is a multi-site mission-driven non-profit organization employing over 700 staff whose primary focus is providing the highest quality patient care to residents throughout the Merrimack Valley. We have seven clinical sites, six pharmacies, two school-based health centers, two mobile health units, and one community services site that provides OBAT. Nationally recognized as a leader in community medicine (family practice, pediatrics, internal medicine, and geriatrics), GLFHC has clinical sites throughout the service area and is the sponsoring organization for the Lawrence Family Medicine Residency program.
This is a full-time role for an ambulatory care Clinical Pharmacist. The Clinical Pharmacist supports and works alongside a multidisciplinary healthcare team and patients to help ensure that the medications prescribed for patients contribute to optimized health outcomes. The Pharmacist utilizes strong critical thinking, problem-solving, and decision-making skills to evaluate drug therapy through clinical assessment, direct patient care involvement, subjective and objective findings relating to patient's responses to drug therapy, and communicates/documents those findings and recommendations.
