This Advanced Practice Provider (APP) will join Castell, Intermountain Health's comprehensive health platform company. Castell is focused on elevating value-based care capabilities with providers, payers, healthcare systems, and accountable care organizations. Castell will deliver impactful solutions that help other organizations accelerate their transition from volume to value-based systems of care, improve outcomes, and keep costs more affordable.
The Advanced Practice Provider will provide primary care to our most at-risk patients (often with comorbidities) in their homes in Northern UT. The APP will be considered a leader for the Castell House Calls team, working with nurse care managers and social workers, and others in a multidisciplinary approach to patient care. Position Specific Duties:

How often? You get to choose how often you work! We offer weekly scheduled shifts, and PRN's for one time coverages.

*This line of care is very dependent on our clients needs. We will do our best to provide consistent set schedules but this is a position where communication is key. Warming up or cooking food
Helping client move or change position
Light housekeeping
Personal hygiene and dressing (ADL's- showers, toileting, etc.)
Errands or appointments
Medication reminders, exercises, etc.
Qualifications and Skills
Adult care experience is required.
A caring demeanor

Valid driver's license and insured car
