Requirements of the Physical Therapy Assistant (PTA):
Must possess, an Associate's Degree from an accredited college or university for Physical Therapy Assistant (PTA).
Must have experience in an assistant capacity in a hospital, nursing care facility, or other related medical facility.
Must possess the ability to interpret the policies and procedures of physical therapy.
Must maintain the care and use of supplies, equipment, etc., and maintain the appearance of therapy areas.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing therapy services.
Must be able to relate information concerning a resident's condition.
Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
Rehabilitation Techs are utilized to assist therapists with resident transport and administrative functions. The rehab department utilizes Optima Therapy for billing and documentation to assist therapists with daily efficiency where quality care remains the primary focus of each therapist. The facility has a 7-day a week, In-House comprehensive rehabilitation program that is an integral member of the interdisciplinary team. Our rehab positions are stable and secure at one location. Each rehab department offers spacious state of the art equipment and ADL Suites.
Responsibilities of the Physical Therapy Assistant (PTA):
Assist in planning, developing, organizing, implementing, evaluating, and directing our physical therapy services as well as its programs and activities, as directed by the Facility's Physical Therapists.
Assist in developing and maintaining physical therapy standards.
Assist in standardizing the methods in which therapy will be accomplished.
Review physical therapy policies, procedures, manuals, job descriptions, etc., at least annually, and participate in making recommended changes.
Assist in interpreting physical therapy policies and procedures to personnel, residents, family members, etc., as necessary.
Participate and assist in discharge planning, development and implementation of resident care plans, resident assessments, etc., as necessary
Maintain a current file of residents who have been treated.
Maintain treatment records, resident files, and progress notes as required.
Ensure resident records are not lost or mishandled.
Ensure the Business Office receives charges for physical therapy rendered to residents.
Maintain and adequate liaison with families and residents.
Interview residents, or family members, as necessary.
Perform administrative requirements such as completing necessary forms, reports, etc., and submitting to the Physical Therapist, as required.

Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies or personnel, and the general public.

Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
Must be able to relate information concerning a resident's condition
Must possess basic computer knowledge

Evaluate performance, initiate disciplinary actions, and prepare and complete performance evaluations for personnel.
Perform nursing assessments and collaborate with the nursing team, other disciplines, patients, and families to develop effective plans of care.
Plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care
Establish realistic, measurable short and long term patient goals with interventions put in place as needs arise to minimize rehospitalizations and continued evaluation of outcomes.
Administer medications and performs treatments per physician orders.
Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented.
Communicate patient information with assigned staff and between shifts.
Function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
Relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the Facility.

Responsibilities of the Speech Language Pathologist (SLP):
The Speech Language Pathologist (SLP) will screen, examine and evaluate residents, including history, systems review, applications of appropriate tests and measures and interprets findings in order to establish a diagnosis, identify impairments, determine the predicted level of improvement and time required to achieve it, identify precautions and design a plan of care.
Develops appropriate treatment goals and methods in collaboration with the resident and caregivers, implements the treatment plan and completes all related documentation required for those services.
Speech Language Pathologist (SLP) will attend and contributes to resident care, staff conferences and other related meetings.
Consults with and makes referrals to other disciplines, agencies in order to address the comprehensive needs of the resident.
Provide exception customer service: ensures that resident and families receive the highest quality of service in caring and compassionate environment.
Speech Language Pathologist (SLP) will be responsible for Clinical Programming.
Responsible for maintaining efficiency standards.
Requirements of the Speech Language Pathologist (SLP):
Must possess, as a minimum, a Bachelor's Degree in Speech Language Pathologist from an accredited college or university.

Spring Creek Rehab and Nursing is seeking motivated and compassionate Licensed Practical Nurses to join our team!
We have Full time and Part-Time shifts available.
Day shift 7am-3pm -part time
Evening Shift 3pm - 11pm - full time and part time
Night Shift 11pm-7am -full time and part time
LPN with trach and vent exp - 12 hour day shift and 12 hour night shift
The LPN is responsible for the following:
Monitoring residents, their responses to treatment care plans.
Supervising and directing.
Document necessary information regarding nursing care, care plans and observation of residents overall conditions.
Perform various duties to provide quality nursing care to residents.
Analyzes information for appropriate interventions to prevent decline.
Record signs of change in condition.
Notify appropriate staff per facility protocol to evaluate weight loss, decline in ambulation, development of skin breakdown, etc.
Administers medications and treatments to residents; monitor administration of intravenous fluids; places orders for medication.
Maintain resident clinical files; keep charts updated; document appropriately.
Make rounds to see each resident daily.
Job Requirements
At least one year of experience in a Long Term Care environment is highly preferred.
Must be a graduate of an approved practical nursing program.
Must possess and maintain a current state PA license as a Licensed Practical Nurse (LPN)
Must be able to move, lift and position patients.
