As a Care Associate, you will operate from your home office while remaining an integral member of a nationwide supportive team dedicated to providing administrative support to the Clinical Teams of Nurses and Vocational Rehabilitation Counselors assigned to high-profile Federal contracts. As a Care Associate you will provide increased efficiency for the operation by providing general support to the medical staff in a team environment The CMU assists clinical staff throughout the entire case management process. You will ensure high standards of service delivery and compliance while playing a crucial role in enhancing patient care.
Care Associate of Central Management Unit Duties and Responsibilities
Act as a liaison of information between the clinical team of SGS, SGS Case Management systems, and the client's electronic records.
Ensure by confirmation that required documents have been received; distribute, fax, mail, and copy incoming and outgoing correspondence.
Monitor and take timely action on assigned worklists
Access and assign cases for clinical professionals based on claim specifics, geographic needs, and caseloads
Enter new claims data into the claims management system accurately; maintain data integrity.
Perform quality assurance reviews according to protocol
Review, organize, attach, and label incoming medical documents, to appropriate case management systems.
Timely data entry for new and existing referrals.
Collaborate with members of the health care team to provide ongoing case management services.
Accurate timely documentation according to protocols in assigned case management systems.
Understand the organization's quality management program and the case management associates role within that program
Is an effective team member; supports the team by pitching in at any level and effectively working across the organization to meet the needs of the business

Care Associate of Central Management Unit Qualifications
GED or High School diploma required
Associate degree (or higher) in clinical or medical health administration or related field preferred
3+ years of industry experience, healthcare network, patient care scheduling, medical assistant, workers' compensation, or similar equivalent combination of education and experience
Medical terminology expertise preferred
Worker's compensation, disability, and/or liability claims processing experience preferred.
Customer Service experience in the medical field preferred
Ability to utilize an Electronic Medical Record or other electronic platform
Demonstrated ability to use Microsoft applications and demonstrate basic electronic technology understanding
Computer literacy, including MS Word, Excel, and Outlook
Ability to pass a background check
Proficiency in Microsoft Office, tech-savvy
Appreciate and comply with all confidentiality requirements
Responsible for having reliable High-Speed Cable or Fiber Optic Internet service and Internet Router in established home office

Remote Nursing Care Attendant Job
PURPOSE AND SCOPE: Responsible for coordination and management of evaluation of kidney transplant patients in the pre-transplant phase. TECN will work in collaboration with their TECN Scheduler to schedule, confirm and execute testing. TECN will review results of testing (see responsibilities) and compile all testing once complete to submit to transplant center. TECN will review all testing and elevate abnormal testing to designated Transplant Center physician. TECN will also collaborate with the Transplant Center team to accomplish pre transplant evaluation where appropriate. TECN will attend meetings with Transplant Center when needed and present patients at selection conference for decisioning on
candidacy.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
* Clinical review of Incoming Referrals & Start Referral Testing per Protocol
* Team meetings with transplant center clinician leaders to confirm permission to order testing prior to being seen 1:1 by clinician.
* Confirm Pre-Evaluation Work-Up Tests Requested by Transplant Center
* Includes securing "signature" from transplant center MD on orders for testing.
* Contact patient and secure "consent for transplant evaluation" (needed to bill the transplant center for testing and services)
* Work out with transplant center: Should patient/family education session by transplant center about risks/benefits of transplantation and orientation to the evaluation process happen prior to securing consent to referral.
* Order missing health screening tests (Txp Neph signature)
* These are orders for testing that is not on the "standard list" of UNOS-required labs and/or not explicitly outlined in transplant center evaluation testing guidelines.
* Schedule health screening tests (hospital services/external partners)
* Need to identify local partners (e.g. local radiology centers, local cardiology clinics) who can undertake necessary testing:
* Ostensibly, CXR, mammogram, CT abd/pelvis to assess vasculature can all be done in same place at the same time.
* ECG, transthoracic echo, and/or stress echo, and/or biochemical-provoked stress test can all be done in same cardiology testing center at the same time.
* Pap smear - q 3 years by current screening requirements.
* Colonoscopy versus Cologuard is a point of discussion with transplant center clinical leadership. Assist patient in scheduling if necessary.
* Dental - Assist patients in obtaining dental Need to work out specific dentist resources particularly for traditional Medicare or dual-eligible patients.
* There may be "special case" scenarios for patients with specific insurance. E.g. some commercially insured patients may require testing to be done at the transplant center parent hospital. Need to develop a clear algorithm with communication pathway with transplant center administrator/billing personnel to address questions.
* Review all results and review abnormal testing with designated Transplant Center physician or designee.
* Organize To/From Transport for Patient
* Ideally, location of radiology and/or cardiology clinic is less important if this is "one off" testing, that is, all testing can be done same place/same day x 1.
* Consider "special transportation" for selected patients. Preference is that much of this transportation can either be via patient themselves, patient-caregiver/friend, and if otherwise not available, via non-critical transportation (e.g. Uber/Lyft). Distinguish between tests that require a "support person" after (e.g. colonoscopy and anesthesia).
* Confirm patient intent/attendance (dialysis clinic SW) TECN will periodically need to communicate/coordinate with FKC clinic MSW about upcoming appointments. TECN will need to communicate with FKC MSW about patients that are proving to be difficult/impossible to schedule/re-schedule -
* Approach for escalation to attending nephrologist to review if patient really is a candidate for referral or whether there is a psychosocial barrier that has been uncovered.
* Coordinate billing & reimbursement for testing with transplant center Financial Coordinator
* Familiarization with key billing codes for testing to ensure this is billed through the transplant center and not billed to the patient.
* Key part of patient consent: Any follow-up testing because of an abnormal test is not part of the transplant evaluation testing and must be billed to the patient's insurance. This could entail a financial obligation for the patient, depending on individual circumstances. But TECN must keep straight initial screening testing (can be coded as such and billed to transplant center) and follow-up testing (which can't).
* TECN needs to be able to participate in periodic audit trails of this documentation.
* Coordinate tracking of test completion, securing and reviewing test results, attach results to original referral from FKC clinic as "single package," deliver and confirm receipt of records by Transplant Center. Coordinate anomaly management & follow-up
* TECN needs tracking system to follow, in near to real time, that a scheduled test was completed (or not), the test was reviewed and resulted, collect the results, review the results for abnormal findings that need prioritized review and disposition by transplant center MD, include all the testing results into a comprehensive "referralplus" packet, and deliver with confirmed receipt to transplant center.
* TECN needs a clear communication pathway with transplant center MD (or designee) for escalating abnormal results for review and disposition.
* Communication of abnormal results to patients needs to be worked out by prior agreement. Since this testing is to be ordered by transplant MD and not attending nephrologist MD, a procedure for how abnormal results and recommended followup evaluations are communicated to the patient needs to be agreed-to beforehand. Since the attending nephrologist has a prior relationship with the patient, enlisting them to play a role here may be well advised.
* Tracking and chasing down missing tests.
* TECN needs password-protected access to Spectra results platform as well as to radiology and cardiology testing partners. Access needs to include visibility into confirming test was ordered, confirming test was completed, and results review.
* Transplant centers will likely want access to actual films. Rather than make TECN responsible for this, it would be preferable to have a routine means of delivering actual films to transplant center for uploading into system. API for transplant related labs between Spectra and transplant center, tagged to ordering MD inbox for review, would be preferred.
* TECN needs clear guidance for how to review patients who repeatedly miss scheduled appointments.
* "Customer relations"
* Arrange lobby/practice days with transplant center personnel.
* Coordinate joint population level reviews between practices, clinics, and transplant centers.
* Solicit feedback from patients, referring physicians/practices, transplant center on TECN operations.
* Provides continuous updates to Operations Management regarding clinical posture of assigned Centers.
* Performs other related duties as assigned.

The position of the Nurse Chronic Care Coordinator, Remote will perform telephonic encounters with patients on behalf of our partners each month and develops detailed care plans within our care plan templates in the electronic health record. This begins as an Independent 1099 Contractor position but offers the potential to reach full-time W2 employment (with employee benefits).
Esrun Health is seeking Nurses to work part-time from their home office while complying with HIPAA privacy laws. You will set your own hours and will not be held to a daily work hour schedule. You will be contracted to work a minimum of 20hrs/wk. Esrun Health wants its team members to have the flexibility to balance their work-life with their home life. Part-time team members will typically need to dedicate an average of 20-30 hours per week to care for their assigned patients. This unique business model allows you to choose what days and what hours of the day you dedicate to care for your patients.
The Care Coordinator will be assigned a patient panel based on skill and efficiency level and is expected to carry a patient panel of a minimum of 100 patients per calendar month. Care Coordinators will be expected to complete encounters on 90 percent of the patients they are assigned.
Esrun Health utilizes a
productivity-based pay structure
and pays $10.00 per completed patient encounter up to 99 encounters/month, $10.25/encounter from 100-149 encounters/month, $12/encounter from 150-199 encounters/month, $14/encounter from 200-249 encounters/month, and $16/encounter for >250 encounters/month. Payment tier increases require 2 months consistency to achieve. 300 Med-Surg Pulmonary is a fast paced, dynamic unit that specializes in Respiratory, Telemetry and Vascular Surgery patients. As a nurse on this unit, you will be trained to deliver high quality care to a complex patient population. Our unit specializes in patients that require telemetry monitoring, cardiac drips, and advanced respiratory support with supplemental oxygenation. This unit is ideal for nurses who aim to enhance their critical thinking skills, master various nursing techniques, and deliver patient centered care. Our unit has a highly functioning team that is supportive of each other and has a strong sense of teamwork. Job Summary
The Medical-Surgical nurse is responsible for managing the care of the adult or geriatric patient experiencing general medical conditions or general surgical procedures. The nurse must be able to assess patient condition, administer medications, change dressings, monitor vital signs, keep records and provide patients and families with support and education. The Medical-Surgical nurse maintains a wide array of medical care knowledge in order to care for a diverse group of patients. The hybrid virtual eAcute nurse will work at least one shift per pay period as an eAcute Virtual Med surg nurse and the remainder of shifts as a bedside nurse. The eAcute Virtual Med-Surg nurse is a pivotal member of the healthcare team to assist with managing the care of the adult or geriatric patient experiencing general medical conditions or general surgical procedures. The virtual nurse will assist with tasks such completing the admission and discharge process, care plan development and maintenance, patient education, medication and discharge teaching, care coordination, mentoring of new nurses, and implementation of evidence-based care. The virtual eAcute nurse assists the primary bedside nurse with nursing tasks not required to be done in person as well as real-time quality and patient safety surveillance. The eAcute Virtual Med-Surg nurse maintains a wide array of medical care knowledge in order to care for a diverse group of patients. Virtual training begins after successful 12-week orientation period as bedside nurse.

Compensation & Benefits

MAJOR FUNCTION The primary purpose of the Post-Graduate Year 2 Ambulatory Care Pharmacy Resident is to work in collaboration with Henry J. Austin Health Center (HJAHC) providers to provide top quality care to its patients and the general community. The resident advises providers on drug usage and dose control. Specifically, the resident will advise providers on any and all dangers of various drugs, list their side effects, and help them determine the correct dosages for the patient. Additionally, the resident will provide direct clinical care to patients including medication therapy management, participation in collaborative practice agreements, coordination of treatment with physicians, and general patient care management. The residency also includes a staffing component and in this context, the resident will serve a traditional retail pharmacist. The resident will verify prescriptions, counsel patients, and perform other staff pharmacist duties as determined by the Pharmacist in Charge. The resident will also perform research and teaching duties as determined by the Residency Advisory Committee (RAC) under the supervision of the Residency Program Director (RPD).
