Provide direct and individualized nursing care to assigned residents based on established nursing protocol. Observe and assess for: pain/discomfort, oxygen therapy, all drainage, all dressings, and any incontinent elimination which may have occurred.
Provide ignition and ongoing systematic assessment of the physical, emotional, social, educational, and functional needs of the resident/family through observation, interview, and physical examination.
Interpret assessment data and information to assist in developing a plan of care.
Assure medical orders are transcribed and processed accurately and timely and integrate the medical care plan into the provision of nursing care.
Assure that medication administration is correct, according to the established nursing policies and procedures.
Provide accurate and timely documentation consistent with the resident plan of care.
Administer medication per physician orders. Document properly on MOR. Observe for any adverse reaction.
Assemble resident charts, resident records, and assure all necessary form are completed in a timely manner. Assure all monthly summaries are completed in a timely manner for assigned residents.
Prepare incident/accident reports for residents as needed per policy, notify AL Director, physician, and family post incident per policy and document accordingly.
Assist resident in learning self-care activities.
Supervise all resident assignments to nursing team members, and assure special instructions are given to team members.
Observe and supervise all meals to ensure residents receive the proper diet and any assistance required.
Monitor and organize all material resources necessary for meeting needs of assigned unit.
Recognize emergencies and life-threatening situations and initiate appropriate action.
Ensure that resident call lights are promptly answered, and their needs are addressed timely and appropriately.
Assist with the orientation of new staff members.
Ensure that all scheduled activities take place on assigned unit by utilizing activity personnel and/or nursing personnel.
Level of Formal Education: Bachelor's Degree from a four-year college or university; or one to two years of related experience and/or training; or equivalent combination of education and experience

The Program
Interim Housing is a county-funded Department of Health Service (DHS) Office of Diversion and Reentry (ODR) program for individuals declared incompetent to stand trial. Instead of receiving services in a state hospital or locked facility, individuals are diverted to receive community based competency restoration. The Interim Housing program provides intensive, wraparound behavioral health services at 4 transitional housing sites.
The Position
The Resident Aide ensures that the interim housing site is a safe, healthy and therapeutic environment. Resident Aides are responsible for assisting clients with increasing independence with activities of daily living and assists clients in adhering to program policies and procedures. This position reports to the Interim Housing Program Supervisor. A successful Resident Aide takes responsibility for the health and safety of our clients and is able to develop strong rapport with clients, encouraging them to engage in treatment.
Essential Functions:
Ensure the interim housing site is a safe and healthy environment
Monitor client activities and traffic in and out of the site. Ensure all residents at the site are accounted for and guests are checked in.
Ensure that clients have access to basic needs. Facilitate meal service and linkage to needed hygiene items.
Serve as a first responder to crisis situations. Actively assist clients in crisis through use of deescalation and problem solving skills. Escalate client issues to supervisor or emergency personnel as appropriate.
Complete basic housekeeping tasks (ie sweeping/mopping, disinfecting surfaces).
Ensure that the housing site is well maintained and supplies are well stocked. Alert Interim Housing Program Supervisor when items are in need of repair and/or supplies need to be ordered.
Support the housing site in maintaining a therapeutic treatment environment
Acts as a role model and guide for clients. Encourage participation in treatment activities and self care tasks. Interactions with clients are consistent with harm reduction, trauma informed and recovery oriented care approaches.
Encourage clients to adhere to program structure and daily routines. Communicate issues to the treatment team.
Assists site in facilitating events and recreational activities for clients.
Assist clients with increasing independence with activities of daily living

**Responsibilities**
As our Nursing Assistant at St. John's you'll help our patients feel safer and more comfortable by assisting with their basic daily life activities.
Your success in this role comes from your genuine desire to help patients feel better whether that means giving them fresh bed linens or checking their vitals. You're diligent about your tasks observant of patients' conditions improving or declining and accept coaching well. You know we're all continuously trying to become better care providers and you're eager to learn.

**Qualifications**
+ High school education
+ Successful completion of Certified Nursing Assistant program

**Preferred Qualifications:**
+ Six (6) months recent hospital experience as a CNA
**Special Skills:**
+ Ability to communicate effectively in English both verbally and writing; clearly concisely and accurately

Job Duties and Responsibilities:
Float to assigned unit/s: Med surg/Tele or ICU
Assists patients with activities of daily living such as feeding, toileting and hygiene
Provides basic nursing care to non-acutely and acutely ill patients
Assists in maintenance of a safe and clean environment
Works under the directions and supervision of the registered nurse in charge of the team and/or unit
Answers phones and call lights
Ensures unit is stocked with supplies
Assists patient transporter as needed
Other duties as assigned
