Initiates, assigns, and assist with activities, as appropriate
Check all assigned residents daily for cleanliness, appropriate clothing, any continence issues and change in condition
Uses proper sanitary procedures and universal precautions
Checks on assigned resident rooms daily for general cleanliness and neatness and takes appropriate action; ensures high risk rooms are double checked
Assist with conducting laundry rooms, common areas including bathrooms checks for cleanliness and any exposed hazardous chemicals; and takes appropriate action (i.e. contacting housekeeping, personally cleaning the room, securing the chemicals)
Completes all ADLs for assigned residents according to the resident's Personalized Service Plan (PSP)
Responds to security system and resident call bells promptly & immediately; takes appropriate action including resetting call bells

The Hospice Aide Under the direction and ongoing supervision of a registered nurse, provides personal direct patient care services as established and delegated in the Plan of Care daily.
Reports after patient encounter to the Executive Director, or designee, regarding the patient's condition or environment, following the Care Management process on as needed basis.

BASIC SKILLS

*Display ability to give personal care with dignity including the areas of: bathing (bed-complete/partial tub or shower), perineal care, oral hygiene (denture care), nail and hair care, dress/undress, prepare and assist with meal service, assist to feed (use of adaptive utensil/devices), assist with bowel/bladder elimination, and care of the dying and post mortem care focused on the utmost respect for the Residents.
*Assist in straightening and organizing Residents' rooms and returning clean linen and laundry.
RESTORATIVE SERVICES
Display ability to perform restorative services including: apply/use of assistive devices for ambulating, eating, and dressing; maintenance of range of motion, proper turning, positioning, transferring, bowel & bladder training, care & use of prosthetic devices.
RESIDENT RIGHTS
*Demonstrate behavior that indicates recognition of Residents right to privacy and confidentiality. Allow for Resident choices, activity involvement, and respect Resident belongings. *Turn and reposition bedfast Residents, alone or with assistance, to prevent bedsores.

*Clean, sterilize, store, prepare, and issues dressing packs, treatment trays, and other supplies.

*Attend in-services as mandated by State/Federal regulations, 12 hours per year or more as required. Attend department or unit meetings.
Have the flexibility with their schedule to meet needs of the department as mandated by State/Federal regulations and Home requirements.
Observe dress and conduct codes, including wearing identification badge, and demonstrates acceptable personal hygiene. Uniform is clean and appropriate, hair net and apron is worn for dining room service.
Consistently be at work on the unit and on time as scheduled.
Follow instructions, responds to management direction; Takes responsibility for own actions; Keeps commitments; Commits to long hours of work when necessary to reach goals.
Perform other duties as assigned by supervisor with an attitude of cooperation and support.
WORK SETTING/ENVIRONMENT:
The Nursing Assistant will perform work primarily indoors, but may attend meetings/seminars outside of the facility or may participate in outdoor activities of the Home. The work environment is well lit, clean and suitable to tasks performed by the employee within temperature control standards that govern a skilled care facility. The potential at risk factor pertains to Category I exposure to blood, body tissue, fluids, with occasional exposure to hazardous materials and bodily injury/illness. Seldom are there exposures to toxins.
EQUIPMENT OPERATED:
The Nursing Assistant will be expected to operate a variety of equipment, e.g. wheelchairs, Geri-chairs, beds, carts, trays, a wristwatch with second hand, blood pressure apparatus & stethoscope, whirlpool tub/lift, telephone, pager, hearing aids, walking devices, chair scale, gait belt, shavers, and computers.

PHYSICAL AND COGNITIVE REQUIREMENTS TO PERFORM THE ESSENTIAL JOB FUNCTIONS:
Nursing Assistants are able to:
Walk and stand constantly - 90% of shift. Frequently climb stairs, bend, squat, kneel, twist, sit, bend and reach from floor to overhead, and at times must move about in tight places and between objects.
Push/pull occupied wheelchairs/Geri-chairs with Residents weighing over 200 pounds.
Transfer, lift and turn a Resident to or from a bed, wheelchair, toilet, tub and shower and occasionally the floor. Varying degrees of pushing, pulling and lifting are required due to differences in Resident weights, which could be over 200 pounds. In emergency situations, must be able to bear the Resident weight and to move the Resident to safety.
Continuously perform simple manipulative tasks as well as have finger, hand, and arm dexterity/mobility and hand/eye coordination to safely provide personal cares, and operate/calibrate some equipment, i.e. instruments to take a blood pressure.
Have sufficient vision to see things up close so as to notice a change in Resident condition, (breathing, color, skin breakdown, etc.). Must have color discrimination to see call lights.
Have adequate hearing to continuously hear normal sounds and voice patterns with some background noise. Must be able to hear/receive verbal instructions, answer the phone and call lights, respond to audible emergency signals/alarms and communicate effectively with Residents and others.
Detect the smell of smoke, spoiled food, soiled linen, etc.
Be able to effectively verbally communicate in English with Residents and others, and speak clearly to report Resident conditions.
Have the cognitive ability to concentrate on moderate detail with some interruption. Attention span must be necessary to attend to a function from 10-25 minutes.
Understand and relate specific ideas and concepts (for example, how items taught during in-service training would be applied when giving care).
Have the memory necessary to remember task/assignment given ranging from short to long periods of time. Must comprehend, remember and follow multiple instructions and directions for a full shift. Frequently must remember and follow directions without reminders over long periods of time.
Have the emotional stability to deal effectively with the stress created by Residents' illnesses, disabilities, the aging process and organizational change. Must show the ability to work cooperatively as part of the health care team.
EMPLOYEE RESPONSIBILITY IN AN EMERGENCY: The Nursing Assistant is expected to respond to emergency situations involving the safety of Residents, other employees and the facility. This includes the ability to assist with a possible evacuation of Residents.
