Job Description
Qualifications:
  • Graduate of accredited school of nursing, currently registered in New York State as a Registered Professional Nurse. Life support and infection control certificate required.
  • At least one year of experience as a staff nurse preferably in an ambulatory care setting preferred. New grads with 3 months' med/surg experience required.
  • Recent experience performing phlebotomy.
  • Computer literate; working knowledge of EHRs
  • Excellent verbal and written communication skills. Excellent and interpersonal skills. Bi-lingual in English and Spanish helpful.
  • Ability to be a team player.
Position Summary:
  • Assumes responsibility and accountability for the provision of nursing care in accordance with relevant federal requirements, state and local laws and nurse practice acts, regulatory standards of professional ambulatory care nursing practice, other relevant professional standards, and organizational policies.
  • Coordinates, implements, evaluates and directs patient care activities, appropriate to the age of patients served and areas of clinical practice, in collaboration with members of the health care team.
  • Supports the mission and philosophy of Care Centers.
Essential Functions:
  • Demonstrates the ability to perform assessments and data collection in a systematic manner, focusing on physiological, psychological, cognitive, age specific and cultural needs of the patients.
  • Consistently utilizes current evidence-based standards of professional practice and standards of care in the assessment and planning process.
  • Demonstrates initiative, competence and enthusiasm for carrying out daily assignments, assisting and taking directions from other health care providers in the delivery of patient care and maintaining patient flow.
  • Collaborate effectively with medical providers and other clinical disciplines in planning interventions that will best meet the patient and family needs in a timely manner.
  • Demonstrates a commitment to maintaining a supportive work environment that fosters collaboration and communication between all health care team members, projects a positive image to the communities we serve and demonstrates respect for the cultural and religious diversities and needs of patients, families and care providers.
  • Ensures that patients receive timely and appropriate care within the delivery system and facilitate internal and external referrals.
  • Utilizes the nursing process to perform patient assessments, triage appropriately and assist the medical provider in the provision of care.
  • Collaborates with medical providers in identifying patient's educational needs and provides the resources to meet these needs.
  • Verbalizes and demonstrates knowledge of policies and procedures and is able to articulate these to other members of the health care team.
  • Demonstrates the ability to delegate tasks appropriately to other members of the care team and utilizes appropriate organizational/priority setting skills and able to assist supportive personnel in the flow of scheduled patients.
  • Performs within scope of practice, demonstrates awareness of legal issues, and utilizes appropriate judgment and actions to reduce potential adverse consequences for patients, self and the organization.
  • Demonstrates initiative and competency in the utilization of Health Information Technology to perform essential job functions, which include but are not limited to (1) Documentation in the EHR, (2) Transmission of EHI, (3) Retrieval of EHI.
  • Communicates with pharmacies as well as insurance companies related to patient treatment plans and approvals of medications and/or procedures.
  • Performs efficiently in emergency situations; remains calm, uses good judgment and follows established protocols.
  • Utilizes appropriate techniques in performing venipuncture to obtain blood specimens in accordance with agency procedure.
  • Verbalizes knowledge of and adheres to OSHA standards for blood-borne pathogens when handling blood and body fluids.
  • Assesses personal learning needs and demonstrates commitment to professional growth by attending continuing education seminars.
  • Provides a safe and secure environment for patients, family, staff and self through the appropriate application of established safety and infection control principles.
  • Assumes role of patient advocate and conforms to policies and regulations governing patient rights, confidentiality and the HIPAA Privacy & Security Rules.
  • Able to verbalize an understanding of the components of the Chronic Care Model and apply these in the planning and delivery of care.
  • Maintains a consistent pattern of attendance in accordance with the assigned schedule and reports to work on time.
  • Conforms to agency guidelines regarding dress code; is well groomed and neat. Maintains good personal hygiene.
  • Performs all related duties, as assigned or unrelated duties, as requested in the presence or absence of the Practice Administrator.
