Nurse - Essential Duties and Responsibilities:
1. Assumes the professional responsibility and accountability for a group of residents/patients including:

c. Supervision of all nursing staff to assure implementation of the interdisciplinary care plan.
2. Performs ongoing and systematic assessments utilizing the nursing process to establish a comprehensive goal directed interdisciplinary plan.
3. Communicates and collaborates effectively with residents/patients, families, colleagues and other members of the interdisciplinary team to assure that treatment is appropriate to meet the age, needs and abilities of resident/patient and unit needs.
4. Documents accurate, pertinent and comprehensive information in a timely and concise manner as required by regulation & facility policy & procedure.
5. Supervises the care provided by CNAs & LPNs assuring adherence to the plan of care, resident profile, policies and procedures.
6. Participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. Attends all annual mandatory programs.
7. Participates in quality improvement activities as directed
Follows standards of the Diligent Minimal Lift Program
9. Performs other duties as required.
Minimum Qualifications:
Education: Associates Degree/Diploma Program, Graduate of accredited school of nursing
Experience: 1-2 years nursing experience in clinical setting preferred. New Grads may be considered. Previous LPN or CNA experience is a plus.
Certificates, Licenses, Registrations:
Current RN llicense in the State of Connecticut
Key Competencies: Ability to work as part of a team and to manage multiple priorities; to speak, read and write English in order to communicate with patients, residents and clinical staff; to effectively interact with residents and staff; organized; ability to utilize a personal computer/CRT to input and retrieve patient information; and familiarity with the Care Plan Development.

Our client, located in Canton, Massachusetts, is a state-operated pediatric chronic care hospital under the Department of Public Health. It provides comprehensive medical, rehabilitative, educational, and recreational services to children and young adults (ages 7–22 ) with multiple disabilities, helping them achieve greater independence. Services include 24/7 nursing care, various therapies, special education, adaptive equipment support, and a range of therapeutic and recreational programs.
The Case Manager coordinates, monitors, develops and implements comprehensive treatment plans for developmentally disabled patients. The Case Manager reviews treatment outcomes and provides communication with state and local agencies to ensure program implementation. Makes decisions through problem solving, instruction of other employees, negotiation and reconciliation of differences with members of the treatment team. Performs other work-related duties as assigned by supervisor.

Leads interdisciplinary treatment team in discharge planning process with knowledge and experience with discharges from hospital environments related to youth transitions to adulthood.

Interview patients and families at a time of pre-admission evaluation to assess appropriateness for admission. Document findings and make recommendations to Admission Committee.

Meets with families of each new patient, obtain a complete psycho-social history which is placed in the patient's medical record.
\t
Reviews and analyzes data on a continuous basis through the care planning process to determine progress by measuring outcomes to make recommendations for changes in procedures affecting guidelines, interventions and approaches that accomplish program/treatment objectives.
\t
Evaluates and analyzes information about patient in order to suggest approaches for resolving legal, family, social, economic, behavioral and/or physical problems. Assists with solving problems, to determine patient's progress and to inform patients about their eligibility status etc.
\t
Able to demonstrate knowledge and skills regarding age specific physiological changes, developmental needs, equipment needs and appropriate therapeutic interventions.
\t
Knowledge of principles of growth and development over the life span and interpret the appropriate information needed to identify each patients requirements related to age specific needs.
\t
Assist patients and families in discharge planning for patient in order to assume a smooth transition in the community upon discharge.
\t
Explore housing, education and medical options in the community with the patient and family in order to provide best possible plan for patient upon discharge.
\t
Participate in patient/family/education in the areas of treatment planning, process and progress, adult service providers and insurance information.
\t
Facilitate regular unit meetings designed to educate patients in financial, community, organization and social skills to achieve highest level of independence in each case.

Identifies opportunities in improving service delivery and participates in developing new processes/solutions.

The Hospice Palliative Care LPN is responsible for and provides skilled nursing care reflective of the agency's mission to the ill or injured in the home, hospital or extended care facility. The LPN works under the direction of the Nurse Case Manager and/or Clinical Manager.
Essential Duties and Responsibilities:
Responsible for coordination of care with community and agency resources and with the designated registered nurse on a regular basis.
Provides nursing care as per the established plan of care and documents clinical activities per the agency policy
Recognizes and interprets symptoms and institutes remedial measures within the scope/limits of a Licensed Practical Nurse in the state of CT and reports significant changes in the patient condition to the primary RN Case Manager/Clinical Manager immediately.
Administers medication and treatments as ordered by the physician, with the exception of IV push medication. LPN must pass competency for intravenous/subcutaneous administration of medication via continuous infusion. (LPN may increase or decrease dose, including delivering bolus dose via patient infusion pump, as per instruction and delegation of duty by the Case Mgr.
Reinforces appropriate interventions, medications, effects and side effects to patient, caregiver, volunteers and other as appropriate as directed by the RN Case Manager.
Implements care to achieve outcomes of comfort, symptom management, safe dying, self-determined life closure and effective grieving for patient and family.
Identifies and addresses comfort care needs
Maintains the dignity of the dying patient.
Supports the patients and family's unique spiritual and cultural beliefs.
Provides holistic, family-centered care across treatment settings to improve the quality of life.
Consults and collaborates with the interdisciplinary team and others involved in the patient's care.
Provide care consistent with National Hospice and Palliative Care Organization standards of practice for hospice programs.
Observations of adult and geriatric client's condition are accurately reported to the designated registered nurse and documented appropriately.
Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements.
Performs other duties as assigned by Hospice Clinical Manager/designee.
Minimum Qualifications:
Education: Graduate of a NLN accredited school of nursing required.
Experience: Experience in home health care or hospice preferred.
