Position Responsibilities: · Provide direct nursing care to patients in clinic, home-based, and community settings. · Conduct clinical assessments, monitor health status, and deliver timely interventions. · Collaborate with primary care physicians, psychiatric providers, social workers, and other team members to develop individualized treatment plans. · Administer medications and treatments as prescribed, ensuring safe and effective implementation. Conduct drug screenings as needed. · Health Education and Preventive Care, educating patients and families on mental health, substance use prevention, chronic disease management, and lifestyle changes. · Provide psychoeducation on depression, anxiety, trauma, and other behavioral health conditions. · Conduct health screenings for diabetes, hypertension, and obesity, offering preventive recommendations. · Coordinate referrals to external providers as needed and collaborate with community partners and resources to support comprehensive patient care. · Ensure continuity of care through follow-up visits and monitoring progress. · Complete accurate and timely documentation in electronic health records within the same day of intervention. · Ensure compliance with organizational policies, procedures, and standards of care, including infection control and safety protocols. · Participate in interdisciplinary team meetings and performance improvement activities. · Maintain effective communication with team members to ensure seamless coordination of patient care. · Support clients through behavior modification, psychoeducation, and referrals for smoking cessation and substance use treatment programs.
