Overview:
We're looking for a Hospice Nurse Practitioner to join our team. You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
Performs and completes physical assessments for the F2F encounter within the regulatory time frames established by CMS
Provides clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility
Makes appropriate recommendations to the IDG based on findings of the F2F assessment
Notifies hospice agency when unable to complete a F2F visit according to schedule
About You:
Master's or higher degree in Nursing.
Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
Current state license as a registered nurse
Current state license or certificate as an advanced practice nurse
Current national certification as a Nurse Practitioner. (ANCC or AANP)
Must have a collaborative agreement in place.

The typical pay range for this role is:
$87,035.00 - $187,460.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

PRIMARY RESPONSIBILITIES
Completes follow-up visits after the initial assessment is completed

Consult with the attending psychiatrist when necessary
Makes referrals for further diagnostic psychological or medical testing as needed
As appropriate, communicate with patients, patients' families, and others to obtain personal and social information pertinent to the patient's treatment
Prepares documentation following Ridgeview Institute's policies and all regulatory and accrediting bodies
Works in collaboration with a multi-disciplinary team to perform clinical assessments

Overview
We're looking for a Hospice Nurse Practitioner to join our team. You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis.
