Job Summary
The Advanced Practice Provider (APP) cares for our patients within the scope of training and approved the agreement, as outlined by their governing board. Works collaboratively with other providers and staff to ensure efficient patient flow and a high level of patient satisfaction. This position works directly with a franchised business location. The specific job duties and benefits can vary between franchises.
Responsibilities
Perform complete, detailed, and accurate health histories, review patient records, develop comprehensive medical assessments, and order laboratory, radiological and diagnostic studies appropriate for complaint, race, sex, and physical condition of the patient
Formulate medical and nursing diagnoses and institute therapy or referrals of patients to the appropriate health care facilities, agencies, and other resources of the community or physician
Institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, convulsions, poisoning, and allergic reactions
Interpret and analyze patient data to determine patient status, patient management and treatment
Provide instructions and guidance regarding health care and health care promotion to patients/family/ significant others
Other duties and responsibilities as assigned

Overview:
We're looking for a PRN Hospice Nurse Practitioner to join our hospice team. You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
Performs and completes physical assessments for the F2F encounter within the regulatory time frames established by CMS
Provides clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility
Makes appropriate recommendations to the IDG based on findings of the F2F assessment
Notifies hospice agency when unable to complete a F2F visit according to schedule
About You:
Master's or higher degree in Nursing.
Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
Current state license as a registered nurse
Current state license or certificate as an advanced practice nurse
Current national certification as a Nurse Practitioner. (ANCC or AANP)
Must have a collaborative agreement in place.

You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
Performs and completes physical assessments for the F2F encounter within the regulatory time frames established by CMS
