Overview:
We're looking for a Hospice Nurse Practitioner to join our team. You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
Performs and completes physical assessments for the F2F encounter within the regulatory time frames established by CMS
Provides clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility
Makes appropriate recommendations to the IDG based on findings of the F2F assessment
Notifies hospice agency when unable to complete a F2F visit according to schedule
About You:
Master's or higher degree in Nursing.
Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
Current state license as a registered nurse
Current state license or certificate as an advanced practice nurse
Current national certification as a Nurse Practitioner. (ANCC or AANP)
Must have a collaborative agreement in place.

YOUR ROLE
As an Oncology Navigation Nurse Practitioner, you will be a critical clinical team member caring for our members. You will have three primary responsibilities:
First, using a telemedicine platform, you will evaluate and manage members identified by our nursing and provider team who have urgent symptoms, complications related to their treatment, or exacerbations of their medical conditions. To do this, you will review available medical records, work with external providers, give clinical insights to our care team, and adhere to best documentation practices to drive high-quality care for our members.
Third, you will be responsible for the oversight and direction of interdisciplinary case conferences for our members, in which individualized care plans are created and monitored by our team of Community Health Workers and Oncology Nurse Navigators. You will provide clinical oversight to the case management team and direct clinical care, as needed.

Assess the needs and status of patients designated by and under the supervision of the physician (licensed MD preceptor); obtaining complete histories of previous health, nutritional history, family history, immunization records, social/personal history, and health summary; must average 250 patients per month.
Systematically perform and record complete general physical examinations, including pelvic examinations and PAP smears
Differentiate between normal findings and abnormal ones, and record history and physical findings in patients' permanent records; the MD Preceptor is responsible for approving and countersigning the recordings of the FNP.
Assess and maintain patients with controlled chronic disorders such as diabetes mellitus, hypertension cardiovascular diseases, chronic brain diseases, chronic joint disorders, chronic lung disease, and minor common acute illnesses.
Prescribes routine medications and immunizations under the orders, protocols and supervision of the MD Preceptor
Screens the results of laboratory tests, ordering further tests as indicated; to assess the patient's needs for health education; instructs patients in the care of specified health problems; and makes appropriate referrals. The Practitioner is to refer acute cases immediately to the physician, including seriously or critically ill patients, complicated diagnostic problems, and patients needing new evaluations and therapy.
