• Assesses physical and psychosocial status of assigned patients as outlined by Nursing Standard Operating Procedures
• Prepares and Provides accurate documentation of the visit in a timely manner
• Provides medical care as needed and with the scope of practice of the Nurse Practitioner
• Actively participates in the coordination of the patient's hospice care
• May be asked to perform routine visits to patient homes and/or participate in the on-call nursing schedule

About You

• Master's or higher degree in Nursing or related field with primary focus on Nursing (licensed in the state of requested employment)
• Completion of graduate level nurse practitioner education program accredited by a national accrediting body and documentation of certification by a national credentialing body
• Must be able to effectively operate nursing assessment tools and office equipment to include, computer and or tablet
• Sensitivity to feeling of the terminally ill and their families before and after patient's death
• Knowledge of terminally ill patients and their families along with understanding of the hospice concept

Works collaboratively with physicians and multi-disciplinary health team. Responsible for providing comprehensive care to patients. **Required:**

+ Current license from Maryland Board of Nursing or Compact State as Registered Nurse (RN).

+ Current license from Maryland Board of Nursing as Nurse Practitioner (NP).

+ Current registration with Drug Enforcement Administration (DEA).

+ Current BLS Healthcare Provider and ACLS Provider certification from American Heart Association.

**Minimum Knowledge, Skills & Abilities Required** **:**

+ Graduate of accredited school of nursing; Master of Science in Nursing (MSN), with advanced practice specialty.

+ Minimum of three(3) years ofrelevant experience in professional nursing role; minimum of two(2) years of experience as nurse practitioner in pain management; medical-surgical, intensive care and perioperative experiencepreferred.

**Overview**

St. Luke's Health System is seeking a Advanced Practitioner to join their highly collegial multidisciplinary Cardiopulmonary Rehabilitation team in Boise, Idaho.

The selected individual will be working to provide coverage in a flex capacity.

Our teams goal is to stabilize, slow, or even reverse the progression of cardiovascular disease. Providing education and counseling support to help our patients adjust their lifestyle behaviors, and help them develop sustainable, healthy habits in exercise, nutrition, and weight management.

ACLS experience is needed.

**JOB REQUIREMENTS INCLUDE:**

+ Graduation from an accredited school with a minimum of a Master's Degree and certification, as required by the specialty, as well as eligibility to obtain licensure from Idaho to practice as a Nurse Practitioner

+ Must obtain and maintain unrestricted prescriptive authority, DEA, and controlled substance licenses.

+ Excellent communication skills to include oral and written comprehension/expression.

+ Ability to manage a fast-paced, busy clinical environment related to changing patient needs, including work with patients with acute, chronic, and complex disease processes.

+ Ability and willingness to exhibit behaviors consistent with standards for performance improvement and organizational values (e.g., efficiency & financial responsibility, safety, partnership & service, teamwork, compassion, integrity, and trust & respect).
