Requirements
1. Bachelor's degree from a certified Nurse Practitioner accredited program is required. Master's degree is preferred.
2. Current licensure as a Nurse Practitioner with the State of California is required. 3. Current Drug Enforcement Agency (DEA) Licensure is required.
4. Current American Heart Association (AHA) Healthcare Provider CPR card is required.
5. Valid American Heart Association Advanced Cardiac Life Support (ACLS) healthcare provider certification is required.

You will:

R EQUIRED QUALIFICATIONS :

Minimum of two years of medically relevant experience or equivalent
Complete the DOT certification exam within 30 days of employment
Depending on the market, the ability to be proficient in both speaking and writing in additional languages not limited to but including Spanish
Basic Life Support (BLS) certification, including but not limited to performing CPR and operating an Automated External Defibrillator (AED).
Pass a FIT test and wear the provided respirator mask when protocol requires .
PREFERRED QUALIFICATIONS:
If less than 1 year of work experience as a licensed provider, prior student precepting rotation with MinuteClinic is preferred, but not required .

The typical pay range for this role is:
$46.03 - $99.14
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

The salary range for this role is $89,800 to $176,700 annually based on full-time employment. Regional Placement:
This position may require the applicant to work at multiple clinics. Our clinics are split into South and North regions
North: Pearland, Houston (Scarsdale Blvd), Alvin, League City, Bay Region- Bacliff, League City, and Pasadena
South: Angleton, Lake Jackson, Clute, and Freeport
This medical provider position may will reside at one of the Southern Region of Clinics. The successful candidate would qualify for the Premium Rate Pay, which is an additional 10% of the base pay
Southern Region includes clinics in Clute and Freeport
Job Summary:
Functions as an advanced practitioner and is responsible for the provision of primary health care for clients of all ages. Prescribes via prescriptive authority. Collaborate with physicians. The Family Nurse Practitioner is committed to ensuring a positive patient experience and following PCMH guidelines and principles, integration of services, and whole person care.
Duties and Responsibilities:
Applies advanced specialty knowledge, clinical skills, and expertise to assess patients and develop a list of actual and potential needs/diagnoses
Responsible for gathering health history of patients through interview of patient and/or family member and physical examination
Identifies risk factors through discrimination of normal and abnormal findings obtained through health history, physical examination, laboratory and/or radiological test results, and behavioral assessment
Applies advanced knowledge, skills, and expertise in planning and implementation of patient's care by:
Assessment and management of wellness with consultation or referral as needed
Referral of unstable, chronic illness
Counseling and education relating to specific health care needs and/or health promoting activities
Collaboration with physicians and other health care professionals in the provision of care
Must maintain professional licenses and professional competency through continuing education
Must maintain patient documentation in a prompt and comprehensive manner.
Ensure patient calls are returned and patient follow-up for labs, referrals and pharmacy are handled promptly
Responsible and accountable for improvement of quality measures with expected positive health outcomes

Required Education and Experience:
Licensed as Family Nurse Practitioner with recognition as Advanced Practice Nurse or licensed as a Physician Assistant with recognition by the Texas Medical Board
Master of Science in Nursing
Certification in area of practice or eligibility for certification
Must possess or be eligible to obtain a DEA license.
Preferred Education and Experience:
ACLS certification desirable
