Overview:
We're looking for a Hospice Nurse Practitioner to join our team. You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
Performs and completes physical assessments for the F2F encounter within the regulatory time frames established by CMS
Provides clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibility
Makes appropriate recommendations to the IDG based on findings of the F2F assessment
Notifies hospice agency when unable to complete a F2F visit according to schedule
About You:
Master's or higher degree in Nursing.
Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body.
Current state license as a registered nurse
Current state license or certificate as an advanced practice nurse
Current national certification as a Nurse Practitioner. (ANCC or AANP)
Must have a collaborative agreement in place.

Ability to communicate effectively in verbal and written form with retail and medical partners at various levels, patients, family members, physicians and representatives of the community
Sound understanding of all federal and state regulations including HIPAA and OSHA
2 or more years of direct patient care required

Nurse Practitioner Job In Carrollton, GA
We are seeking an APP for Tanner Health's General Cardiology service based in the Stress testing lab. The provider will join a multi provider team of physicians and other APP's to provide stress test to patients as ordered by referring providers. This position will provide services at Tanner Medical Center Carrollton every other weekend. This APP will work collaboratively within a multidisciplinary healthcare team to provide Stress Testing services for patients with Cardiac issues under the supervision of the Non-Invasive Cardiology attending physician.

Position Requirements
* Hours. Your work schedule shall include, without limitation, applying your skill and knowledge as a part-time NP two (2) eight (8) hour shifts, or at least sixteen (16) hours, every other week on Saturday and Sunday.
Job Eligibility Requirements
Education
* Completed Physician Assistant program in good standing or Master of Nursing with specialization in Nurse Practitioner
Certification/License
* Currently hold or be able to obtain a Georgia Nurse Practitioner or Physician Assistant License
* Currently hold or be able to obtain a Board Certification as Nurse Practitioner
Experience
* Open to New graduates or candidates with experience in related Cardiac field.
Qualifications
* Able to communicate effectively with medical and hospital staff and various populations in clinics and public screening events.
* ACLS recommended.
* Additional certification will be required for some specialties (i.e. ACLS, PALS)
* Graduate of a recognized Physician Assistant program.
* Must be capable of dealing tactfully and effectively with patients, family members, other employees and physicians
* Must maintain high degree of confidentiality.
* Must meet credentialing criteria.
* Must possess the cognitive ability to understand patient medical records, physician orders and medical orders to be communicated to patient, hospital and/or other physician.
* Must possess the sense or discipline to work in accordance with accepted clinical standards.
* Prior experience in a like specialty preferred.
