    SUMMARY: As part of a collaborative agreement with a physician as mandated by Ohio law and under the direction of the AxessPointe Community Health Center, Inc. (API) Medical Director, the NP/PA will provide quality primary medical and behavioral health services to adults, children and families in a holistic manner within the northeast Ohio area. SUPERVISOR: Medical Director or Designee DUTIES AND RESPONSIBILITIES: * Examines, diagnoses, and treats illnesses and injuries of patients throughout the lifespan. * Provides well-child, well-woman and well-man health maintenance exams to new and established patients. * Orders appropriate screening and diagnostic lab and radiographic exams. * Interprets results of physical exams and screening/diagnostic tests to implement appropriate care plans for the patient. * Prescribes medicines, therapeutic agents and treatment modalities as required by patient needs and in accordance with licensed prescriptive authority. * Supports health promotion, health education, health empowerment and disease prevention interventions for all patients. * Provides age-appropriate care to all patients. * Promotes patient and family centered care through a collaborative team approach. * Participates in established call group as necessary. * Refers patients to specialty practices as appropriate and needed. Provides medical care to patient as part of mutual patient care with specialist. * Confers with collaborating physician about patient care as necessary. * Documents and maintains patient records of services provided according to program standards. * May instruct medical students or nursing students. * Operates within a scope of practice as agreed to by the FNP/PA, the collaborating physician, the medical director and in compliance with Ohio state law. * Supervision and Administrative experience helpful * You may be asked to perform any of the job duties within your respected scope of practice at any AxessPointe location. Requirements KNOWLEDGE, SKILLS AND ABILITIES: * Knowledge of principles and practices of preventative medicine. * Knowledge of state and federal laws pertaining to medicine and public health. * Formal MSN-NP/ PA training and certification in the state of Ohio to practice medicine. * Knowledge of the structure and function of community health centers. * Skill in communicating effectively with patients and families to include culturally sensitive care. * Skill in establishing and maintaining effective working relationships with other employees, patients and the general public. * Commitment to AxessPointe mission, vision and values. MINIMUM QUALIFICATIONS: * Graduation from an accredited MSN/ARC-PA program * Bilingual a plus LICENSES OR CERTIFICATIONS REQUIRED: * Board certification from ANCC/NCCPA or other valid NP/PA accreditation body with a focus in family medicine (all ages) * Licensed in the state of Ohio * Current DEA Registration COMPLIANCE: This position requires compliance with API's written standards, including its Compliance Program and Standards of Conduct and policies and procedures.
